Pharmacodynamic Properties: Human menopausal gonadotropin (hMG) acts directly on the ovaries and testes and has both gametotropic and steroidogenic effects. In women, its follicle-stimulating hormone (FSH) component promotes the growth and development of ovarian follicles. FSH also stimulates estradiol production in granulosa cells, while the luteinizing hormone (LH) component supports ovarian steroid production.
In men, FSH promotes the maturation of Sertoli cells and supports the development of sperm-producing structures in the testes. Adequate testosterone levels are also necessary for sperm production, which may be achieved through prior hCG treatment.
Pharmacokinetic Properties: Gonagen is not effective when taken orally and is administered by intramuscular or subcutaneous injection. Its biological activity comes primarily from its FSH and LH components. After intramuscular administration, peak FSH levels are generally reached within 6–48 hours; after subcutaneous administration, they are reached within 6–36 hours. The hormone is subsequently eliminated, mainly through the kidneys.

The dose is individualized according to ovarian response and should be adjusted based on ultrasound examinations and serum estradiol measurements. Treatment generally begins with 75–150 IU of FSH daily, administered intramuscularly or subcutaneously.
If the ovaries do not respond adequately, the dose may be increased gradually under specialist supervision. Once sufficient follicular development is achieved, hCG may be administered to trigger ovulation. If estradiol levels rise too rapidly, the dose may need to be reduced.
Excessive stimulation can cause multiple follicle development and increase the risk of ovarian hyperstimulation syndrome (OHSS), particularly following hCG administration.
Dosage in Men
Treatment generally begins with hCG administered twice weekly to stimulate testosterone production. Once testosterone levels reach the desired range, Gonagen may be added to stimulate sperm production. The dose and duration depend on the patient's hormonal response and should be determined by a specialist.
Specific drug interactions have not been well established. Gonagen may be administered with hCG in men undergoing infertility treatment when prescribed by a specialist. Inform your doctor about all medicines and supplements you use.
