Common:

Dizziness, fatigue, headache

Nausea, diarrhea, constipation

Cold extremities, bradycardia

Less Common / Rare:

Shortness of breath, hypotension

Depression, sleep disturbances

Bronchospasm (in predisposed patients)

Worsening heart failure (rare)

Metabolic effects: ↓HDL, ↑triglycerides, may mask hypoglycemia

✔️ Dosage & Administration | Probis 5

Adults

Hypertension / Angina: Starting dose 5 mg once daily. If needed, increase to 10 mg, then up to 20 mg once daily.

Heart Failure:

• Week 1: 1.25 mg once daily
• Week 2: 2.5 mg once daily
• Week 3: 3.75 mg once daily
• Week 4: 5 mg once daily
• Week 8: 7.5 mg once daily
• Week 12: 10 mg once daily (max dose)

• Hepatic impairment (severe): Start at 2.5 mg/day; max 10 mg/day.
• Renal impairment (CrCl <40 ml/min): Start at 2.5 mg/day.
• Elderly: 2.5–20 mg/day (Hypertension), up to 10 mg/day (Heart failure).

Administration:

• May be taken with or without food.
• Take regularly at the same time each day.
• Swallow whole with water; do not crush or chew.

✔️ Contraindications:

• Cardiogenic shock
• Overt cardiac failure
• Second or third degree AV block
• Marked sinus bradycardia
• Severe hypotension

✔️ Interaction

• Class I antiarrhythmics (quinidine, disopyramide, propafenone): Increased AV conduction delay.
• Catecholamine-depleting drugs (reserpine, guanethidine): Risk of excessive sympathetic suppression.
• Clonidine: Avoid abrupt withdrawal (may worsen rebound hypertension).
• Digitalis glycosides: Increased risk of bradycardia.
• NSAIDs: May reduce the antihypertensive effect.
• Verapamil/Diltiazem: Additive myocardial depression and AV block.

✔️ Precautions & Warnings

• Use with caution in renal/hepatic impairment.
• May mask symptoms of hypoglycemia in diabetic patients.
• Avoid abrupt discontinuation – taper gradually to prevent rebound hypertension or angina.
• Not recommended in severe peripheral arterial disease or severe asthma/COPD.
• Use with caution during pregnancy (Category C) and lactation.
