✔️ Pregnancy & Lactation

Pregnancy:

• Studies in animals suggest that Bisoprolol is not teratogenic at doses up to 375 times the maximum human dose. However, it showed fetal toxicity at higher doses.
• Use during pregnancy only if the potential benefit outweighs the risk to the fetus.

Lactation:

• Bisoprolol passes into animal milk in small amounts (<2%). It is unclear if it is excreted in human milk. Nursing mothers should discontinue breastfeeding if Bisoprolol use is necessary.

✔️ Dosage & Administration

Adults:
For mild to moderate hypertension, the dosage should be tailored to the individual:

• Start with 5 mg once daily, either alone or combined with a diuretic.
• If 5 mg is insufficient, increase to 10 mg daily, and if necessary, to 20 mg daily.
• Allow at least two weeks between dose adjustments. Doses above 20 mg daily offer limited additional benefits.

Children:
The safety and efficacy of Bisoprolol in children have not been established.

Renal or Hepatic Impairment:

• For patients with liver dysfunction (e.g., hepatitis or cirrhosis) or renal impairment (creatinine clearance <40 mL/min), start with 5 mg daily.
• Titrate doses cautiously to avoid drug accumulation. Bisoprolol is not dialyzable, so replacement is unnecessary for dialysis patients.

Elderly:
Dose adjustments are generally not required unless significant renal or hepatic impairment is present.

Use this medication as prescribed by a registered physician.

✔️ Side effects may vary among individuals and include:

• Fatigue, dizziness, headache
• Gastrointestinal disturbances such as nausea, vomiting, diarrhea, constipation, or abdominal pain
• Cold or numb extremities (e.g., hands and feet)
• Muscle weakness or cramps
• Bradycardia (slower than normal heart rate)
• Bronchospasms cause breathing difficulties, particularly in people with asthma or COPD

✔️ Interactions

• Other β-Blockers: Avoid combining Bisocor with other beta-blockers.
• Catecholamine-Depleting Drugs: Medications like reserpine or guanethidine may enhance the sympathetic-blocking effect of Bisocor, requiring close monitoring.
• Centrally Acting Antihypertensives: Caution is needed when combining Bisocor with drugs like clonidine due to the risk of rebound hypertension. If discontinuing clonidine, stop Bisocor several days before to avoid exacerbation.
• Antiarrhythmic Agents: Use Bisocor carefully with myocardial depressants or A-V conduction inhibitors, such as certain calcium channel blockers (e.g., verapamil, diltiazem) or antiarrhythmics (e.g., disopyramide).
• Calcium Channel Blockers: The combination of Bisocor and calcium channel blockers with negative inotropic effects may lead to bradycardia, severe hypotension, or cardiac failure in patients with impaired ventricular function.

✔️ Bisocor should not be used in patients with:

• Cardiogenic shock
• Overt heart failure
• Second or third-degree A-V block
• Right ventricular failure due to pulmonary hypertension
• Sinus bradycardia

✔️ Precautions & Warnings

• Use with caution in patients with renal or hepatic impairment.
• Patients with a history of severe anaphylactic reactions may have heightened sensitivity to allergens and may not respond to usual epinephrine doses.
