  • Tetrabenazine reversibly inhibits human vesicular monoamine transporter type 2 (VMAT2), which leads to decreased uptake of monoamines (such as dopamine, serotonin, norepinephrine, and histamine) into synaptic vesicles.

    • The maximum recommended single dose is 25 mg.
    • If adverse reactions occur, such as akathisia, restlessness, parkinsonism, depression, insomnia, anxiety, or sedation, stop titration and reduce the dose.

Dosing Recommendations Above 50 mg per day:

  • Patients requiring doses greater than 50 mg per day should be genotyped to determine their CYP2D6 metabolizer status (poor metabolizers or extensive metabolizers).
  • Dose individualization based on metabolizer status:
    • Extensive and intermediate metabolizers (EMs or IMs) should be titrated up slowly at weekly intervals by 12.5 mg daily.
    • Doses above 50 mg per day should be given in a three times-a-day regimen.
    • The maximum recommended daily dose is 100 mg, and the maximum recommended single dose is 37.5 mg.
    • If adverse reactions occur, such as akathisia, parkinsonism, depression, insomnia, anxiety, or sedation, stop titration and reduce the dose.

  • Consider withdrawal of Tetrabenazine or the initiation of other specific treatments (e.g., antidepressants) if adverse reactions persist.
