✔️ Pharmacology of Taven EZ 10/10

Atorvastatin: This statin works by inhibiting HMG-CoA reductase, an enzyme involved in cholesterol synthesis in the liver. It helps reduce plasma cholesterol by promoting hepatic uptake and degradation of LDL through upregulation of LDL receptors.

Ezetimibe: This agent reduces cholesterol absorption in the small intestine by targeting the sterol transporter NPC1L1. This inhibition results in reduced cholesterol delivery to the liver, leading to lower hepatic cholesterol stores and increased clearance of circulating cholesterol. Its mechanism complements that of statins, without affecting the synthesis of cholesterol or bile acid excretion.

✔️ Dosage & Administration of Taven EZ 10/10

Typical dosage ranges from 10/10 mg to 80/20 mg once daily.

The initial dose is usually 10/10 mg or 20/10 mg once daily, with 40/10 mg recommended for those needing a significant LDL-C reduction (≥55%).

Dose adjustments should be based on lipid profile assessment done at least 2 weeks after starting or changing the dose.

• For HoFH Patients: A daily dose of 40/10 mg or 80/10 mg is advised, used with other lipid-lowering therapies if available.
• Hepatic impairment: Contraindicated in active liver disease or unexplained high transaminase levels.
• Renal impairment: While no dosage adjustment is needed, monitor patients closely for muscle-related side effects.

• Pediatric use: Not established.
• Geriatric use: No dose adjustment required.

The likelihood of muscle damage (myopathy/rhabdomyolysis) increases when statins are used with:

• Fibrates (e.g., gemfibrozil) – avoid co-use.

• Cyclosporine, clarithromycin, HIV protease inhibitors, itraconazole – avoid co-use with cyclosporine; others raise risk.
• Digoxin – monitor levels.
• Oral contraceptives – consider increased levels of norethindrone and ethinyl estradiol.

• Warfarin – monitor INR when initiating or discontinuing Taven EZ.
