✔️ Common Side Effects

• Gastrointestinal: nausea, vomiting, abdominal pain, diarrhea, constipation
• Headache, dizziness, fatigue
• Edema, hypertension
• Menstrual irregularities, decreased libido
• Rash, pruritus, skin hypersensitivity
• Blurred vision, hearing loss
• Liver enzyme abnormalities (rare: hepatitis, liver failure)
• Potentially serious: heart failure, pulmonary edema, QT prolongation

✔️ Dosage + Administration

Adult Dosage (Oral)

• Oropharyngeal candidiasis: 100 mg/day × 15 days
• Vulvovaginal candidiasis: 200 mg twice daily × 1 day
• Pityriasis versicolor: 200 mg/day × 7 days
• Tinea corporis/cruris: 100 mg/day × 15 days
• Tinea pedis/manuum: 100 mg/day × 30 days OR 200 mg twice daily × 7 days
• Onychomycosis: 200 mg/day for 3 months
• Systemic infections: 100–200 mg daily; up to 200 mg twice daily for severe cases
• Prophylaxis (AIDS/neutropenic patients): 200 mg/day, may increase to 200 mg twice daily

Pediatric Dosage

• 10 mg/kg/day, max 200 mg/day
• For chronic mucocutaneous candidiasis: 5 mg/kg/day
• <3 years: Safety not established

1. Must be taken with food, preferably after a full meal.
2. Should be swallowed whole—do not chew or crush.
3. Dosage and duration depend on infection type and severity.

✔️ Interaction

• Increases plasma levels of: oral anticoagulants, digoxin, corticosteroids, benzodiazepines, HIV protease inhibitors, macrolides
• Decreases plasma levels with: rifampicin, phenytoin, carbamazepine, isoniazid, PPIs, H2 blockers, antacids
• Risk of QT prolongation/torsades de pointes with astemizole, cisapride, dofetilide, etc.
• Can increase negative inotropic effects with calcium channel blockers (e.g., verapamil)
• Potentially fatal respiratory depression with fentanyl

✔️ Contraindications:

• Hypersensitivity to azole antifungals
• Pregnancy and lactation
• Severe liver disease
• IV formulation contraindicated in CrCl <30 mL/min

✔️ Precautions

• Monitor liver function with long-term use
• Use with caution in CHF, renal insufficiency, and COPD
• Drug passes into breast milk—risk vs. benefit must be assessed
