Alfumax ER 10mg Tablet

✔️ Contraindications:

• Severe hepatic impairment
• History of postural hypotension or micturition syncope
• Intestinal obstruction
• Severe renal insufficiency
• Lactation

✔️ Dosage & Administration | Alfumax ER

Adults

Immediate-release: 2.5 mg three times daily

Maximum dose: 10 mg per day

The first dose should be taken at bedtime to minimize postural hypotension

Extended-release formulation: 10 mg once daily

Elderly Patients

Initially, 2.5 mg twice daily

Dose may be adjusted based on clinical response and tolerability

Acute Urinary Retention

Extended-release: 10 mg once daily

Initiate on the first day of catheterization

Duration: 3–4 days

Hepatic Impairment

Mild to moderate: Start with 2.5 mg once daily; may increase to 2.5 mg twice daily if needed

Severe hepatic impairment: Contraindicated

Renal Impairment

Start with 2.5 mg twice daily

Adjust dose according to patient response and tolerability

Method of Administration

Take with food

Tablets should be swallowed whole

Extended-release tablets:

Take immediately after the same meal each day

Do not chew, crush, or break

Pregnancy Category

Not indicated for use in women

✔️ Use caution in elderly patients and those with:

• Hypertension
• Coronary artery disease
• Renal or hepatic impairment
• Discontinue therapy if angina develops
• Monitor blood pressure regularly due to risk of hypotension
• May impair the ability to drive or operate machinery

• Dizziness (5.7%)
• Headache (3%)
• Fatigue (2.7%)
• Upper respiratory tract infection (3%)
• Abdominal pain, back pain, bronchitis, constipation, dyspepsia, nausea, pharyngitis, sinusitis, impotence (1–2%)

✔️ Drug Interactions

Additive QT interval prolongation when combined with QT-prolonging drugs

Enhanced cardiovascular effects when used with:

Other α1-adrenergic blockers

Atenolol or diltiazem

Increased plasma concentration with cimetidine

Potentially fatal interaction:

Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, ritonavir) significantly increases alfuzosin levels
