Rapasin is prescribed for the treatment of signs and symptoms of benign prostatic hyperplasia (BPH). It is not intended for the management of hypertension.

Silodosin, the active ingredient in Rapasin, is a selective antagonist of post-synaptic alpha-1 adrenoreceptors found in the prostate, bladder base, bladder neck, prostatic capsule, and prostatic urethra. By blocking these receptors, Rapasin induces smooth muscle relaxation, leading to improved urine flow and reduced BPH symptoms.

✔️ Dosage & Administration of Rapasin 8

Standard Dose: 8 mg orally once daily with a meal.
For Moderate Renal Impairment (CrCl 30-50 mL/min): 4 mg once daily with a meal.
Always take as directed by a registered physician.

✔️ Drug Interactions

P-glycoprotein inhibitors (e.g., cyclosporine): May increase Rapasin levels in the blood.
PDE5 inhibitors combined with Rapasin may cause symptomatic hypotension.

✔️ Overdose Management

High doses (up to 48 mg/day) can cause postural hypotension.
Treatment:
- Lie patient in a supine position to restore blood pressure.
- Administer IV fluids if necessary.
- Vasopressors may be used in severe cases.
- Monitor renal function.
- Dialysis is unlikely to help due to high protein binding (97%).

✔️ Elderly Patients

Clinical trials show no significant difference in effectiveness or safety between older and younger patients.
Orthostatic hypotension risk:
- 2.3% in patients under 65 years (vs. 1.2% for placebo).
- 2.9% in patients 65+ years (vs. 1.9% for placebo).
- 5.0% in patients 75+ years (vs. 0% for placebo)

✔️ Hepatic Impairment

Severe hepatic impairment (Child-Pugh score >10): Contraindicated.
Mild to moderate impairment: No dose adjustment required.

✔️ Renal Impairment

Severe impairment (CrCl <30 mL/min): Contraindicated.
Moderate impairment (CrCl 30-50 mL/min): Reduce dose to 4 mg daily.
Mild impairment (CrCl 50-80 mL/min): No dose adjustment needed.

✔️ Contraindications

Severe renal or hepatic impairment.
Concurrent use with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, itraconazole, ritonavir).
Hypersensitivity to Silodosin.

✔️ Side Effects

Most common: Retrograde ejaculation, dizziness, diarrhea, orthostatic hypotension, headache, nasal congestion, and nasopharyngitis.

✔️ Pregnancy & Lactation

Pregnancy Category B.
Not intended for women.
Animal studies show no significant teratogenic effects, but Silodosin should not be used during pregnancy or lactation.

✔️ Precautions & Warnings

Risk of postural hypotension: Patients may experience dizziness upon starting treatment.
Should not be combined with other alpha-blockers.
Cataract surgery patients should inform their ophthalmologist due to the risk of Intraoperative Floppy Iris Syndrome (IFIS).
