Lactu is a hyperosmolar laxative used effectively for the management of constipation. It contains lactulose, a non-absorbable disaccharide that draws water into the bowel lumen, leading to stool softening and stimulation of bowel movements through increased intestinal distention.

Lactulose is a synthetic disaccharide that undergoes metabolism in the colon by saccharolytic bacteria, producing low-molecular-weight organic acids (mainly lactic acid). These acids reduce colonic pH and retain water via osmotic action, thereby enhancing peristaltic activity and promoting defecation.
Since lactulose is minimally absorbed, its systemic pharmacokinetics are not relevant to its therapeutic effect.

For Chronic Constipation:

Adults: Initially 3–6 teaspoons daily; for maintenance, 1½–6 teaspoons daily.
Children (up to 14 years): Initially 3 teaspoons daily; maintenance dose 1–2 teaspoons daily.
Infants & Toddlers: Initially 1–2 teaspoons daily; maintenance dose 1 teaspoon daily.

For Intestinal Flora Disturbances:

Adults: 1–2 teaspoons daily.
Children: 1 teaspoon daily.

For Hyperammonemia:

In Hepatopathy: Up to 18–30 teaspoons daily.

In Portal-Systemic Encephalopathy: 6–9 teaspoons hourly until rapid laxation occurs, then reduce the dose as needed to maintain the desired effect.

Mild flatulence, abdominal cramps, or discomfort may occur at the start of therapy, usually resolving with dose adjustment. Overdosage may cause diarrhea and electrolyte imbalance (mainly potassium loss) if misused.

No clinically significant drug interactions have been reported. However, excessive use may cause potassium depletion, potentially enhancing the effects of cardiac glycosides.
