✔️ Composition: Vilanti

• 25/100 mcg capsule: Contains Vilanterol 25 mcg (as Vilanterol Trifenatate INN) and Fluticasone Furoate 100 mcg.
• 25/200 mcg capsule: Contains Vilanterol 25 mcg (as Vilanterol Trifenatate INN) and Fluticasone Furoate 200 mcg.

✔️ Vilanti combines two types of medication with distinct effects:

1. LABA (Vilanterol): Activates beta-2 receptors in the lungs, stimulating an enzyme that increases cyclic AMP levels in cells, causing the bronchial muscles to relax. This helps to ease airflow obstruction.
2. Corticosteroid (Fluticasone Furoate): Reduces inflammation by acting on different types of cells involved in inflammatory responses, such as mast cells, neutrophils, and lymphocytes, as well as on inflammatory mediators like histamines and cytokines. Although the exact mechanism for treating COPD and asthma isn’t fully known, this combination addresses both the inflammation and airway constriction associated with these conditions.

✔️ Side Effects of Vilanti 100

For COPD:

• Common side effects (incidence ≥3%) include nasal inflammation, upper respiratory infections, headaches, oral candidiasis, back pain, pneumonia, bronchitis, sinus infections, coughing, throat discomfort, joint pain, hypertension, flu symptoms, and fever.

For Asthma:

• Common side effects (incidence ≥2%) include nasal inflammation, oral candidiasis, headache, flu symptoms, upper respiratory infections, bronchitis, sinus infections, throat discomfort, voice changes, and cough.

✔️ Special Population Use: Vilanti 100

• Children and Adolescents: Not recommended for use in patients under 18 years of age.
• Liver Impairment: Patients with moderate or severe liver impairment may experience increased systemic effects of fluticasone. Monitor closely for systemic corticosteroid effects.

✔️ Dosage and Administration

• Administration: For inhalation only—do not swallow the capsule. Place the capsule in the inhaler device just before use. After inhaling, rinse your mouth with water (do not swallow) to minimize the risk of mouth or throat infections.
• Recommended Dosage:
  • COPD: One inhalation of the 25/100 mcg dose daily.
  • Asthma: One inhalation of either the 25/100 mcg or 25/200 mcg dose daily, as prescribed by the physician.

✔️ Drug Interactions

• Cytochrome P450 3A4 inhibitors (e.g., ketoconazole): May increase systemic corticosteroid and cardiovascular effects, so caution is advised.
• Monoamine oxidase inhibitors (MAOIs) and tricyclic antidepressants: May enhance the effects of vilanterol on the vascular system; use with caution.
• Beta-blockers: These may counteract the bronchodilatory effects of beta-agonists and cause severe bronchospasm.
• Diuretics: When used with non-potassium-sparing diuretics, there may be an increased risk of low potassium levels and electrocardiographic changes.

✔️ Contraindications

• Acute Treatment: Not suitable for the primary treatment of status asthmaticus or acute COPD or asthma exacerbations.
• Allergies: Should not be used by those with severe hypersensitivity to milk proteins or any other ingredients.

✔️ Warnings and Precautions

• Use may increase the risk of mouth and throat infections (e.g., Candida albicans). Patients are advised to rinse their mouths after inhalation.
• There’s a higher risk of pneumonia in COPD patients; watch for symptoms.
• Use cautiously in patients with a history of tuberculosis, fungal or bacterial infections, viral infections, or ocular herpes simplex.
• Transitioning from systemic corticosteroids may risk adrenal suppression; taper off systemic steroids slowly. Hypercorticism can also occur with high dosages or in susceptible individuals.
• If this occurs, discontinue immediately and seek an alternative.
• Monitor periodically for bone density loss.
• Long-term use of inhaled corticosteroids (ICS) may lead to cataracts or glaucoma; consider an eye exam if symptoms develop.
• Patients with diabetes should monitor for raised blood glucose levels.
• Be aware of the potential for low potassium levels.
