✔️ Indications:

Deflazacort, a synthetic glucocorticoid, suppresses inflammation by inhibiting the Phospholipase A2 enzyme, reducing prostaglandin synthesis. It also decreases the release of immune mediators, providing immunosuppressive benefits in autoimmune and inflammatory diseases.

✔️ Possible Side Effects

• Gastrointestinal: Nausea, indigestion
• Musculoskeletal: Osteoporosis, myopathy
• Endocrine/Metabolic: Adrenal suppression, hyperglycemia
• Neuropsychiatric: Mood changes, psychosis
• Skin: Acne, thinning, striae, delayed wound healing
• Others: Thromboembolism, infection risk, eye complications (e.g., cataracts, glaucoma)

✔️ Dosage & Administration

Adults:

Acute conditions: Up to 120 mg/day initially

Maintenance dose: 3–18 mg/day

Rheumatoid arthritis: 3–18 mg/day based on response

Acute asthma attacks: 48–72 mg/day, tapered after control

Chronic asthma: Titrate to the lowest effective dose

Dosage equivalence: 6 mg Deflazacort ≈ 5 mg Prednisone/Prednisolone

Children:

General pediatric dosing: 0.25–1.5 mg/kg/day

Juvenile arthritis: 0.25–1.0 mg/kg/day

Nephrotic syndrome: Start with 1.5 mg/kg/day, taper based on response

Asthma: 0.25–1.0 mg/kg every other day

Withdrawal:
Tapering is essential for those using >9 mg/day for over 3 weeks. Reduce dose gradually to prevent adrenal insufficiency and disease flare.

✔️ Drug Interactions

• Enzyme inducers (e.g., rifampicin, carbamazepine, phenytoin) may lower Deflazacort levels—dose adjustment may be needed
• Enzyme inhibitors (e.g., ketoconazole) may increase drug levels—consider dose reduction

✔️ Contraindications

• Hypersensitivity to Deflazacort or excipients
• Recent or ongoing live virus vaccination

✔️ Monitor patients closely if they have:

• Cardiovascular disease, hypertension, or thromboembolic risks
• GI disorders: Ulcers, diverticulitis, or recent GI surgery
• Endocrine/metabolic risks: Diabetes, hypothyroidism, osteoporosis
• Neurologic or psychiatric history: Epilepsy, emotional instability
• Eye diseases: Especially ocular herpes simplex
• Liver impairment: May increase drug levels—dose adjustment may be necessary
