✔️ Side Effects of Cordef Suspension

• Gastrointestinal upset
• Muscle weakness and skeletal effects
• Endocrine disturbances (including adrenal suppression)
• Mood and psychiatric changes
• Eye complications
• Fluid retention and electrolyte imbalance
• Increased infection risk and delayed wound healing
• Skin thinning, acne, striae, and telangiectasia
• Rare cardiovascular or thromboembolic events

✔️ Dosage & Administration of Cordef Suspension

Adults:

• Acute severe conditions: up to 120 mg/day initially
• Maintenance therapy: usually 3–18 mg/day, depending on disease control
• Rheumatoid arthritis: lowest effective dose within the 3–18 mg/day range
• Asthma exacerbation: 48–72 mg/day initially, then tapered; maintenance adjusted to minimal effective dose
• Dose is individualized based on response; generally converted from prednisone using a potency ratio (5 mg prednisone ≈ 6 mg deflazacort)

Children:

• Usual range: 0.25–1.5 mg/kg/day
• Juvenile arthritis: 0.25–1.0 mg/kg/day
• Nephrotic syndrome: initial 1.5 mg/kg/day, then tapered
• Asthma: 0.25–1.0 mg/kg on alternate days may be used

Withdrawal:
Long-term therapy must be tapered gradually to avoid adrenal suppression, especially if used beyond 3 weeks at supraphysiological doses.

• Hepatic impairment: may increase drug levels → dose adjustment required
• Renal impairment: standard corticosteroid precautions apply
• Elderly: higher risk of adverse effects; careful monitoring required

✔️ Interaction

• Enzyme inducers (e.g., rifampicin, carbamazepine, phenytoin, phenobarbitone) may reduce the effect → dose may need to be increased
• Enzyme inhibitors (e.g., ketoconazole) may increase drug levels → dose reduction may be required

✔️ Contraindications

• Hypersensitivity to deflazacort or formulation components
• Patients receiving live virus vaccines

✔️ Pregnancy & Lactation

• Crosses the placenta; prolonged use may affect fetal growth
• Use only when the benefits outweigh the risks
• Small doses (<50 mg/day) are unlikely to affect nursing infants significantly
• Higher doses may cause temporary adrenal suppression in infants

✔️ Extra caution is needed in patients with:

• Cardiovascular disease, hypertension, or heart failure
• Diabetes mellitus
• Peptic ulcer disease or gastrointestinal inflammation
• Osteoporosis
• Renal or liver impairment
• Psychiatric illness or epilepsy
• Thyroid disorders
• Ocular herpes simplex
