Treatment of Parkinson’s disease

✔️ Mechanism of Action

• Levodopa is a precursor to dopamine. It crosses the blood-brain barrier and is converted into dopamine within the brain to help manage movement disorders.
• Carbidopa prevents the premature breakdown of levodopa in the peripheral tissues, increasing the amount of levodopa that reaches the brain for dopamine production.

✔️ Supadopa TR 145 contains two active ingredients:

• Levodopa is a precursor to dopamine converted in the brain to help regulate movement.
• Carbidopa – A decarboxylase inhibitor that prevents levodopa from breaking down outside the brain, allowing more of it to reach the brain and increase dopamine levels.

✔️ Dosage Guidelines for Supadopa TR 145 Adult Dosage

For Parkinson’s Disease & Parkinson-Like Disorders:

Immediate-Release Formulation:

• Initial dose: 25 mg/100 mg every 8 hours or 10 mg/100 mg every 6–8 hours
• Levodopa dosage may be increased by 100 mg per day every 1–2 days
• Carbidopa intake should be within 70–100 mg per day, not exceeding 200 mg per day
• Levodopa should not exceed 800 mg per day

Controlled-Release (CR) Tablet:

• Initial dose: 50 mg/200 mg every 12 hours
• Maximum levodopa dose: 1,600 mg per day
• Doses should be spaced at least 6 hours apart

For Hepatic Impairment:

• Safety and effectiveness have not been well established; use with caution

Pediatric Dosage

• Safety and efficacy in children have not been established

Renal Dosage Adjustments

• For Renal Impairment:

✔️ Supadopa TR 145 should not be used in individuals with:

• Lactation (breastfeeding)
• Narrow-angle glaucoma
• Melanoma
• Psychosis
• Severely decompensated endocrine disorders

Take this medication as prescribed by your doctor. Swallow the capsule whole—do not chew, crush, or break it. It should be taken on an empty stomach. Avoid high-protein foods such as cheese, eggs, milk, and protein powders, as they may reduce the drug’s absorption.

✔️ Drug Interactions

• Decreased Effectiveness of Levodopa: Pyridoxine (Vitamin B6) reverses the effects of levodopa.
• Synergistic Effects: Type-B MAO inhibitors enhance the effects of levodopa.
• Reduced Effectiveness Due to Other Medications: Phenothiazines, haloperidol, reserpine, pyridoxine, diazepam, oxazepam, chlordiazepoxide, and phenobarbital.
• Enhanced Effects of Levodopa: Carbidopa, amantadine, anticholinergics, and amphetamines.
• Enhanced Effects of Sympathomimetic Agents.
• Potentially Fatal Interactions:
  • Severe hypotension with bethanidine, bretylium, and guanethidine.
  • Hypertensive crisis with furazolidone or monoamine oxidase (MAO) inhibitors.

✔️ Pregnancy & Lactation

• Pregnancy Category C: Use only if the potential benefits justify the risks.
• Lactation: This medication may reduce milk production; use with caution.
