✔️ Side Effects

Common side effects include dyskinesia, falls, nausea, and insomnia.

✔️ Dosage & Administration

• Initial dosage: Start with 50 mg orally once daily at the same time each day. If a dose is missed, take the next dose at the same time the following day.
• Maintenance dosage: After 2 weeks, increase to 100 mg once daily based on individual response and tolerability. If discontinuing 100 mg, reduce the dose to 50 mg for one week before stopping.
• Use in children: The safety and effectiveness of Safinamide in infants and children have not been established.
• Geriatric use: No significant differences in safety or effectiveness between older and younger patients.
• Hepatic impairment: Safinamide plasma concentrations may be elevated in patients with hepatic impairment.
• Use as per the guidance of a registered healthcare professional.

✔️ Interaction

• Selective Serotonin Reuptake Inhibitors (SSRIs): Monitor for serotonin syndrome.
• Sympathomimetic medications: Monitor for hypertension.
• Tyramine-containing foods: Risk of severe hypertension.
• Substrates of Breast Cancer Resistance Protein (BCRP): May increase plasma concentrations of BCRP substrates.

✔️ Safinamide is contraindicated in patients with:

• Concomitant use of other monoamine oxidase inhibitors or potent MAO inhibitors (e.g., linezolid).
• Opioids (e.g., tramadol, meperidine), SNRIs, tricyclic or tetracyclic antidepressants, cyclobenzaprine, methylphenidate, amphetamine, their derivatives, or St. John’s Wort.
• Dextromethorphan.
• History of hypersensitivity to safinamide.
• Severe hepatic impairment (Child-Pugh C).

✔️ Pregnancy & Lactation

• Pregnancy Category C: There are no adequate studies in pregnant women. Animal studies show developmental toxicity, including teratogenic effects.
• It is unknown if Safinamide is excreted in human milk. Due to the potential for serious adverse effects in nursing infants, a decision should be made whether to discontinue breastfeeding or the medication, considering the drug’s importance to the mother.

✔️ Precautions & Warnings

• May cause or worsen hypertension.
• Risk of serotonin syndrome when combined with MAO inhibitors, antidepressants, or opioids.
• Risk of falling asleep during daily activities.
• May exacerbate dyskinesia, consider adjusting the levodopa dose.
• Can cause hallucinations, psychotic behavior, or impulse control issues.
• May lead to withdrawal-emergent hyperpyrexia and confusion.
