✔️ Dosage & Administration
Dosage should be tailored to individual patient response and the severity of anginal symptoms.
Adults: One 2.6 mg sustained-release tablet or capsule in the morning and evening, preferably on an empty stomach.
✔️ Side Effects
- Headache (common at the start, often transient)
- Tachycardia, postural hypotension, syncope
- Rare: Cyanosis, methaemoglobinaemia
✔️ Drug Interactions
- Potentiates antihypertensive effects of vasodilators, calcium channel blockers, and beta-blockers.
- Aspirin may enhance hemodynamic effects and reduce the clearance of nitroglycerin.
- May reduce the efficacy of heparin when given together.
- Increases the bioavailability of dihydroergotamine.
- Risk of severe hypotension when combined with:
- Phosphodiesterase type-5 inhibitors (e.g., sildenafil, tadalafil, vardenafil)
- Tricyclic antidepressants (TCAs) and alcohol
✔️ Nitrocontin is contraindicated in patients with:
- Allergy to nitroglycerin, organic nitrates, or excipients
- Acute myocardial infarction
- Marked anaemia
- Head trauma or cerebral hemorrhage
- Closed-angle glaucoma
✔️ Pregnancy & Lactation
- Safety during pregnancy and breastfeeding is not well established.
- Use only if clearly necessary and prescribed by a physician.
✔️ Precautions & Warnings
Use cautiously in patients at risk of closed-angle glaucoma.
- Do not stop treatment abruptly; taper dose gradually under supervision to avoid rebound angina.
- In acute MI, careful hemodynamic monitoring is essential to avoid hypotension and reflex tachycardia.
