✔️ Adverse Effects

Common side effects include:

• Joint pain (arthralgia)
• Nausea
• Localized pain at the injection site
• Dizziness or low blood pressure (especially after initial doses)

✔️ Dosage & Administration

Recommended Dose (for all indications):
20 mcg administered subcutaneously once daily.

Indications include:

Postmenopausal osteoporosis.

Primary or hypogonadal osteoporosis in men.

Glucocorticoid-induced osteoporosis in both sexes.

Route:
Subcutaneous injection into the thigh or abdomen.
Patients should be advised to sit or lie down when receiving their initial doses, due to the risk of orthostatic hypotension.

Pediatric Use:
Not approved for use in children or adolescents, as safety and efficacy have not been established.

Hepatic Impairment:
No specific studies available—use with caution.

Note: Always inspect the solution before use. It should appear clear and colorless. Do not use if discolored or containing particles.

✔️ Drug Interactions

• Digoxin: Teriparatide may temporarily increase calcium levels, potentially altering digoxin's cardiac effects. Use cautiously and monitor closely.
• Hydrochlorothiazide: No significant interaction at 25 mg, but higher doses remain untested.
• Furosemide: Co-administration may modestly elevate serum and urine calcium; not considered clinically significant.

✔️ Contraindications

Known hypersensitivity to teriparatide or any formulation component (e.g., past reactions like angioedema or anaphylaxis).

Patients with:

• Paget’s disease of bone.
• Bone metastases or a history of skeletal malignancy.
• Metabolic bone diseases other than osteoporosis.
• Hypercalcemia or disorders of calcium metabolism.
• Prior skeletal radiation therapy.

✔️ Precautions & Warnings

• Duration of Use: Teriparatide use is not recommended beyond 2 years over a patient's lifetime.
• Risk of Hypercalcemia: Monitor serum calcium levels; transient elevation can occur post-administration.
• Hyperuricemia: May raise serum uric acid—caution in patients with gout or uric acid stones.
• Urolithiasis: Use cautiously in individuals with current or past kidney stones.
• Orthostatic Hypotension: Initial doses may cause dizziness or a drop in blood pressure.
• Lab Monitoring: Monitor calcium, uric acid, and urinary calcium periodically.

✔️ Use in Pregnancy & Lactation

• Pregnancy: Data in humans are insufficient. Use is not advised unless clearly necessary. Discontinue upon pregnancy detection.
• Lactation: Unknown if excreted in breast milk. Effects on the breastfed infant or milk production are unknown—avoid use during breastfeeding.

✔️ Storage Conditions

• Store the pre-filled Teripen delivery device in a refrigerator between 2°C to 8°C.
• Do not freeze. Discard if frozen.
• Keep the pen away from direct light and moisture.
• Ensure the product is used within 28 days after first use.
