✔️ Precautions & Warnings

Renal effects:
Prolonged NSAID use may cause renal papillary necrosis and other kidney abnormalities. Reports of interstitial nephritis, proteinuria, and nephrotic syndrome have been noted. Napgin should be used cautiously in patients with impaired renal function, and renal monitoring is advised.
Avoid use in patients with creatinine clearance <20 mL/min due to risk of accumulation.

Hepatic effects:
Occasional cases of moderate to severe jaundice have been reported. Monitor liver function in long-term therapy.

✔️ Dosage & Administration

Napgin should be taken with water, preferably after meals to minimize gastric irritation.

Naproxen Enteric-Coated Tablets (Adults):

Rheumatoid arthritis / Osteoarthritis / Ankylosing spondylitis: 500–1000 mg daily in two divided doses (every 12 hours). The maintenance dose is 500 mg daily, divided every 12 hours. Do not exceed 1000 mg per day.

Acute gout: 750 mg initially, followed by 250 mg every 8 hours until symptoms subside.

Primary dysmenorrhea: 500 mg initially, then 250 mg every 6–8 hours as needed (for up to 5 days).

Pain and musculoskeletal disorders: 500 mg initially, then 250 mg every 6–8 hours as required.

Children under 16 years: Not recommended for tablet form.

Naproxen Suspension (Children over 2 years):
Used for juvenile rheumatoid arthritis – 10 mg/kg/day in two divided doses (every 12 hours).
Example dosing guide:

13 kg → 2.5 mL twice daily

25 kg → 5 mL twice daily

38 kg → 7.5 mL twice daily

✔️ Drug Interactions

• ACE inhibitors: May reduce antihypertensive effects.
• Antacids & Sucralfate: Delay absorption of naproxen.
• Aspirin: Increases risk of adverse effects.
• Diuretics (e.g., Furosemide, Thiazides): Reduced natriuretic effect.
• Methotrexate: Enhanced toxicity.
• Warfarin and SSRIs: Increased risk of gastrointestinal bleeding.

✔️ Napgin is contraindicated in patients with:

• Known hypersensitivity to naproxen or other NSAIDs
• History of asthma, urticaria, or allergic reactions following NSAID use
• Perioperative pain associated with coronary artery bypass graft (CABG) surgery

✔️ Side Effects

Commonly reported adverse effects include:

• Gastrointestinal: Heartburn, nausea, abdominal discomfort, diarrhea, dyspepsia
• Central nervous system: Headache, dizziness, drowsiness
• Skin: Itching (pruritus), purpura
• Cardiovascular: Palpitations, edema
• Others: Visual or hearing disturbances

✔️ Pregnancy & Lactation

• Naproxen and other NSAIDs can adversely affect fetal development and may delay labor. It has been linked to premature closure of the ductus arteriosus and pulmonary hypertension in newborns.
• Use during the first and third trimesters should be limited to cases where benefits outweigh risks.
• A small amount is excreted in breast milk; while some sources consider it safe, manufacturers often recommend avoiding use during breastfeeding.
