✔️ Dosage Guidelines

For Patients Not Currently on an Erythropoiesis-Stimulating Agent (ESA):

• Hb < 9 g/dL → 4 mg once daily
• Hb 9–10 g/dL → 2 mg once daily
• Hb > 10 g/dL → 1 mg once daily

For Patients Switching from an ESA:

• If previously receiving Epoetin alfa ≤ 2,000 units/week → 4 mg daily
• If previously receiving Epoetin alfa 2,000–10,000 units/week → 6 mg daily
• If previously receiving Epoetin alfa > 10,000 units/week → 8 mg daily
• If previously receiving Epoetin alfa ≥ 20,000 units/week → 12 mg daily

Maximum Dose: 24 mg once daily

• Pediatrics: Not recommended for children.
• Renal Impairment:
  • No significant impact on drug exposure in patients with different levels of renal impairment.
  • Hemodialysis & Peritoneal Dialysis: Drug exposure remains stable.
  • Non-dialysis days: Higher exposure to drug metabolites than on dialysis days.

✔️ Possible Side Effects

• Increased risk of hospitalization due to heart failure
• High blood pressure (hypertension)
• Damage to the gastrointestinal lining, including the stomach, esophagus, and intestines

✔️ Administration & Dosage

• Oral (to be swallowed whole without crushing, chewing, or breaking)
• Can be taken with or without food

✔️ Daprotab 6 should not be used in patients who:

• Are receiving strong CYP2C8 inhibitors (e.g., gemfibrozil), as these can significantly increase drug exposure.
• Have uncontrolled hypertension.

✔️ Missed Dose Instructions

If a dose is missed, take it as soon as possible. However, if it is almost time for the next dose, skip the missed dose and continue with the regular schedule. Do not double the dose.

✔️ Drug Interactions

• CYP2C8 Inhibitors (e.g., gemfibrozil) → Contraindicated due to a marked increase in drug exposure.
• CYP2C8 Inducers (e.g., rifampin) → May decrease drug levels, leading to reduced efficacy.

✔️ Pregnancy & Lactation

• Not recommended during pregnancy due to evidence of fetal adverse effects in animal studies.

✔️ Storage:

Keep below 30°C temperature, protected from light & moisture. Keep out of the reach of children.
