Tezolid 200 IV Infusion

✔️ Dosage & Administration

📌 Adults (18+ years)

200 mg once daily for 6 days

Can be taken orally with or without food, or by IV infusion over 1 hour

If a dose is missed, take it up to 8 hours before the next dose

No dosage adjustment needed when switching between oral and IV forms

Not recommended for patients under 18 years

Use in Elderly

No dose adjustments needed. Pharmacokinetic profiles are similar in elderly and younger adults

✔️ Tezolid is indicated for adults (18 years and older) with ABSSSI due to the following pathogens:

• Staphylococcus aureus (including MRSA and MSSA)
• Streptococcus pyogenes
• Streptococcus agalactiae
• Streptococcus anginosus group (S. anginosus, S. intermedius, S. constellatus)
• Enterococcus faecalis

✔️ Mechanism of Action

Tezolid is a prodrug (Tedizolid Phosphate) that is rapidly converted into its active form Tedizolid in the body. It works by inhibiting bacterial protein synthesis through binding to the 50S ribosomal subunit, leading to bacterial death.

• High oral bioavailability (~91%) makes switching between IV and oral forms seamless.
• Steady-state concentration achieved in 3 days;
• Half-life ~12 hours – suitable for once-daily dosing.
• Primarily excreted via feces (82%) and urine (18%)
• Not metabolized by liver enzymes (CYP450) – low interaction potential

✔️ Reconstitution & IV Infusion Instructions

• Reconstitute each vial with 4 mL of normal saline
• Gently swirl to dissolve – avoid shaking or foaming
• Inspect for particulates and ensure complete dissolution
• Further dilute in an IV bag with 0.9% Sodium Chloride
• Infuse over 1 hour
• Use within 24 hours at room temperature or under refrigeration (2°C to 8°C)

✔️ Contraindications

Known allergy or hypersensitivity to Tedizolid Phosphate or any formulation component

✔️ Possible Side Effects

• Nausea, vomiting, diarrhea
• Headache, dizziness
• Infusion-related reactions (if IV)
• Anemia, blurred vision
• Palpitations, skin rash, pruritus
• Drug hypersensitivity, insomnia, urticaria

✔️ Drug Interactions

Low interaction risk with CYP450 or drug transporters

Reversible MAO inhibitor activity – avoid with serotonergic or adrenergic agents if possible

Monitor patients with psychiatric medications or pressors

✔️ Precautions & Warnings

Use cautiously in neutropenic patients; alternative therapies may be preferred

Monitor for Clostridium difficile-associated diarrhea

Risk of antibiotic resistance with inappropriate or prolonged use

Visual impairment, anemia, hypertension, and CNS effects like dizziness may occur

Avoid in known hypersensitivity to Tedizolid or its excipients

Pregnancy Category C

Use only if the potential benefit justifies the risk

Unknown if excreted in human milk; use with caution during breastfeeding

• Store in a cool, dry place below 30°C
• Protect from light and moisture
• Keep out of reach of children
• After reconstitution and dilution, use within 24 hours
