• Reduces blood pressure effectively, particularly in cases of moderate to severe hypertension.

✔️ Pharmacology

Treatment should be individualized based on response.

• Initial therapy: 5/20 mg once daily for 1–2 weeks, then adjust as needed
• Maximum dose: 10/40 mg once daily
• Elderly patients (≥75 years) may require lower amlodipine doses; therefore, this combination is generally not recommended as starting therapy in this age group
• Pediatric use: safety not established
• Elderly: generally no major differences in response
• Renal impairment: limited data available
• Hepatic impairment: not recommended as initial therapy

Dose adjustments may be made using individual components if required

• Headache
• Dizziness
• Flushing
• Peripheral edema

✔️ Drug Interaction

• NSAIDs (including COX-2 inhibitors) may reduce the antihypertensive effect of olmesartan
• Patients should be monitored for blood pressure control, kidney function, and electrolytes when used with drugs affecting the renin-angiotensin system

✔️ Contraindications

• Not to be used together with aliskiren in diabetic patients

✔️ Pregnancy & Lactation

This combination is classified as Pregnancy Category D. It should not be used during the second and third trimesters due to risk of fetal harm or death. The drug should be discontinued immediately once pregnancy is confirmed.
It is unknown whether the components pass into breast milk; therefore, a decision should be made whether to stop breastfeeding or discontinue therapy.

✔️ Caution is advised in patients with:

• Low blood volume or salt depletion (risk of hypotension)
• Severe aortic stenosis (risk of excessive vasodilation)
• Severe coronary artery disease (may worsen angina or myocardial infarction)
