Xeldrin contains Omeprazole, a benzimidazole derivative that inhibits gastric acid secretion. It works by blocking the hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system in gastric parietal cells. Following oral intake, its effect begins within an hour, peaks within two hours, and lasts up to 72 hours. Once discontinued, normal acid secretion gradually resumes within 3 to 5 days.

- NSAID-associated gastric and duodenal ulcers
- Prevention in patients with a history of NSAID-related ulcers
- Gastroesophageal reflux disease (GERD)
- Long-term management of acid reflux disease
- Acid-related indigestion (dyspepsia)
- Severe ulcerative reflux esophagitis
- Prevention of acid aspiration during general anesthesia
- Zollinger-Ellison syndrome
- Peptic ulcers caused by Helicobacter pylori infection

✔️ Dosage: Intravenous Administration
- Acid aspiration prevention: 40 mg IV slowly over 5 minutes, one hour before surgery.
- Gastric ulcer, duodenal ulcer, or reflux esophagitis: When oral treatment is not possible, 40 mg IV once daily.
- Zollinger-Ellison Syndrome: Initial dose of 60 mg IV daily, adjustable as needed. If exceeding 60 mg, administer in divided doses.
✔️ Administration Instructions:
- IV Injection: Dissolve Omeprazole powder in 10 ml of sterile water and administer slowly over 2-5 minutes at a rate of 4 ml/min. Use freshly prepared solutions within 4 hours.
- IV Infusion: Dissolve the vial contents in 100 ml saline or 5% dextrose and infuse over 20-30 minutes. If using saline, the solution remains stable for 12 hours; if using dextrose, it must be used within 6 hours. Do not mix with other drugs.
✔️ Xeldrin is generally well tolerated, with mild and reversible side effects, including:
- Common: Skin rash, itching, headache, diarrhea, nausea, flatulence, abdominal pain.
- Rare: Photosensitivity, alopecia, dizziness, dry mouth, stomatitis, muscle/joint pain, peripheral edema, increased sweating, vision disturbances, and taste changes.
- Serious (but rare): Blood disorders (leukopenia, thrombocytopenia, pancytopenia), anaphylactic shock, severe liver reactions, and interstitial nephritis.
✔️ Drug Interactions
- May reduce the absorption of Ketoconazole.
- May slow the elimination of Diazepam, Phenytoin, and Warfarin, necessitating monitoring and potential dose adjustments.
- Concomitant use with Clarithromycin increases both drugs' plasma concentrations, which is beneficial for H. pylori eradication.
- No significant interactions with Theophylline, Caffeine, Propranolol, Cyclosporin, Lidocaine, and certain NSAIDs (Piroxicam, Diclofenac, Naproxen).
- Increases Digoxin bioavailability by 10%.
- Concomitant use with Clopidogrel should be avoided as it may reduce its efficacy.
✔️ Contraindications
- Hypersensitivity to Omeprazole.
- If gastric ulcer malignancy is suspected, confirm diagnosis before treatment as Omeprazole may mask symptoms and delay detection.
✔️ Pregnancy & Lactation
- Studies indicate no harmful effects on pregnancy or fetal health.
- Uncertain if Omeprazole is excreted in breast milk; breastfeeding should be discontinued if treatment is necessary.
✔️ Precautions & Warnings
- Long-term PPI use may increase the risk of osteoporosis-related fractures.
- Atrophic gastritis has been observed in prolonged use.
- Concomitant use with Methotrexate may lead to toxicity.'
- Safety in children under 18 has not been well established.
