✔️ Mechanism of Action

Lotil 20 (omeprazole) inhibits the final step in gastric acid secretion by selectively blocking the H+/K+ ATPase enzyme (proton pump) on gastric parietal cells, significantly decreasing acid production—both basal and stimulated.

✔️ Words of Advice:

• Avoid late-night meals and spicy/fatty foods.
• Don’t stop taking Lotil 20 without your doctor’s advice.
• Seek medical help if you develop persistent diarrhea, fever, or kidney-related symptoms.
• Ensure adequate intake of calcium and magnesium if using long-term.

Common Side Effects

• Headache
• Nausea or vomiting

Serious/Rare Side Effects

• Kidney issues: Decreased urination, back pain, fatigue
• Bone fractures (hip, wrist, spine) with long-term use
• Magnesium deficiency: Muscle cramps, irregular heartbeat
• Allergic reactions: Rash, swelling, anaphylaxis
• Liver issues, gastric polyps, or gastritis
• Interstitial nephritis, pancreatitis, or toxic epidermal necrolysis

✔️ Dosage & Administration of Lotil

Adult Dosage

Peptic Ulcer: 20–40 mg daily for 4–8 weeks.

GERD: 20 mg/day for 4–8 weeks. Maintenance: 10–20 mg daily.

Erosive Esophagitis: 20 mg/day for 4–8 weeks; maintenance for up to 12 months.

NSAID-related Ulcers: 20 mg/day.

Zollinger-Ellison Syndrome: Start with 60 mg/day, adjust up to 120 mg/day in divided doses.

H. pylori Infection: 20 mg twice daily or 40 mg once daily in triple or dual therapy regimens.

Pediatric Dosage

1–16 years:

5–10 kg: 5 mg once daily

10–20 kg: 10 mg once daily

20 kg: 20 mg once daily

Erosive Esophagitis in infants (1 month–1 year):

3–5 kg: 2.5 mg/day

5–10 kg: 5 mg/day

10 kg: 10 mg/day (up to 6 weeks)

✔️ Administration of Lotil

• Take once daily in the morning on an empty stomach, preferably one hour before meals.
• Do not crush or chew the capsules; swallow whole. For those with difficulty swallowing, the contents can be mixed with a spoonful of applesauce and swallowed without chewing.

✔️ Interaction

• Increased effects: Diazepam, phenytoin, methotrexate, clarithromycin
• Decreased effect: Clopidogrel (antiplatelet)
• Risk of toxicity: Digoxin, warfarin, benzodiazepines
• Reduced efficacy: Rilpivirine, nelfinavir, atazanavir (HIV meds)
• Decreased absorption: Iron salts, ketoconazole, itraconazole, dasatinib

• Rule out gastric cancer before starting treatment.
• Caution in patients with liver impairment or osteoporosis.
• Pregnant or breastfeeding women should consult their doctor first.
• Monitor magnesium levels during prolonged use.

✔️ Pregnancy & Breastfeeding

• Human studies are limited, but data don’t show major risks in the first trimester.
• Omeprazole passes into breast milk in small amounts; the effect on infants is unclear.
• Animal studies show potential harm at very high doses.
