Pantoprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by irreversibly binding to the H+/K+ ATPase enzyme system located on gastric parietal cells. This action blocks the final step of acid production and effectively reduces both basal and stimulated gastric acid secretion for more than 24 hours, regardless of the triggering stimulus.

- NSAID-induced gastric or duodenal ulcers
- Eradication of Helicobacter pylori infection (in combination with antibiotics)

✔️ Dosage & Administration
Benign Gastric Ulcer
- 40 mg once daily in the morning for 4 weeks
- Treatment may continue for an additional 4 weeks if healing is incomplete
Gastroesophageal Reflux Disease (GERD)
- 20–40 mg once daily in the morning for 4 weeks
- May continue for another 4 weeks if required
- Maintenance dose: 20 mg daily, which may be increased to 40 mg daily if necessary
Duodenal Ulcer
- 40 mg once daily in the morning for 2 weeks
- Continue for another 2 weeks if not completely healed
Helicobacter pylori Eradication
- Pantoprazole 40 mg twice daily combined with:
- Amoxicillin 1 g + Clarithromycin 500 mg twice daily for 1 week
or - Clarithromycin 250 mg + Metronidazole 400 mg twice daily for 1 week
- Amoxicillin 1 g + Clarithromycin 500 mg twice daily for 1 week
Prevention of NSAID-Associated Ulcers
- 20 mg once daily during long-term NSAID therapy
Zollinger-Ellison Syndrome
- Initially 80 mg once daily
- Dose adjusted according to response
- Doses above 80 mg should be given in divided doses

Pregnancy
Pantoprazole is classified as US FDA Pregnancy Category B. Although animal studies have not demonstrated harm, adequate controlled studies in pregnant women are lacking. Use during pregnancy only if clearly necessary.
Lactation
Pantoprazole is excreted into breast milk. A decision should be made whether to discontinue breastfeeding or discontinue the medication, considering the benefit to the mother.

- Tablets should not be split, crushed, or chewed.
- Long-term therapy may reduce vitamin B12 absorption.
- Prolonged use may increase the risk of osteoporosis-related fractures.
- Use cautiously in patients requiring extended treatment duration.
