✔️ Dosage and Administration of Pansec 40

Oral Administration

• Benign Gastric Ulcers: 40 mg once daily in the morning for 4 weeks, with an additional 4 weeks if healing is incomplete.
• GERD: 20-40 mg once daily in the morning for 4 weeks, extendable by another 4 weeks if required. Maintenance dose: 20 mg daily, which can be increased to 40 mg.
• Duodenal Ulcers: 40 mg daily in the morning for 2 weeks, extendable by another 2 weeks if necessary.
• H. pylori Eradication:
  • 40 mg twice daily with:
    • Amoxicillin (1 g) and Clarithromycin (500 mg) twice daily for one week, or
    • Clarithromycin (250 mg) and Metronidazole (400 mg) twice daily for one week.
• NSAID-Associated Ulcers: Prophylactic dose: 20 mg daily for patients requiring long-term NSAID therapy.
• Zollinger-Ellison Syndrome: Initial dose of 80 mg daily, adjusted based on patient response. Elderly patients should not exceed 40 mg daily.

Common Side Effects (1-10%):

• Headache (>4%)
• Abdominal pain (4%)
• Diarrhea (4%)
• Constipation (<4%)
• Nausea and vomiting (>4%)
• Rash (4%)
• Pruritus (4%)

Rare Side Effects:

• Hyperglycemia (1%)
• Flatulence (<4%)
• Photosensitivity (<2%)
• Facial or generalized edema (<4%)

Serious Reactions:

• Angioedema, Stevens-Johnson syndrome, and Toxic Epidermal Necrolysis: Rare but potentially fatal hypersensitivity reactions.
• Pancytopenia and Interstitial Nephritis: Requires close monitoring.
• Hepatocellular Damage: May lead to liver failure in rare cases.
• Pancreatitis, Rhabdomyolysis: Rare but serious adverse events.

✔️ Special Populations

• Elderly: No dosage adjustment is required unless clinically indicated.
• Hepatic Impairment: Limit dose to 20 mg/day or 40 mg every other day.
• Renal Impairment: No dosage adjustment is necessary.

✔️ Drug Interactions

• Increased Risk of Hypomagnesemia: With diuretics.
• Delayed Absorption: Co-administration with sucralfate can reduce the absorption of pantoprazole.
• Warfarin: May increase INR and prothrombin time, necessitating monitoring.
• Methotrexate and Saquinavir: May increase serum concentrations.
• Ketoconazole and Itraconazole: Decreased absorption.
• Rilpivirine, Atazanavir, Nelfinavir: May significantly reduce serum levels and therapeutic effects.

✔️ Contraindications

• Known hypersensitivity to any formulation component.
• Concomitant use with rilpivirine, atazanavir, or nelfinavir.

✔️ Gastric Malignancy:

• Rule out malignancy before starting treatment, as symptoms may mask cancer.

✔️ Long-Term Use Risks:

• May lead to Vitamin B12 deficiency due to impaired absorption.
• Can increase the risk of osteoporosis-related fracture

✔️ Pregnancy and Lactation

• Pregnancy: Classified as Category B. No evidence of harm in animal studies, but use only if necessary.
• Lactation: Pantoprazole is excreted in breast milk. Consider discontinuing either the drug or breastfeeding based on clinical need.
