Mechanism: Blocks ergosterol synthesis by inhibiting lanosterol 14-α-demethylase (CYP450 enzyme).

✅ Description:

Osapiderm is indicated for patients aged 13 years and above in the treatment of:

Invasive Aspergillosis – in patients refractory or intolerant to Amphotericin B or Itraconazole.
Fusariosis – in patients refractory or intolerant to Amphotericin B.
Chromoblastomycosis and Mycetoma – in patients refractory or intolerant to Itraconazole.
Coccidioidomycosis – in patients refractory or intolerant to Amphotericin B, Itraconazole, or Fluconazole, or who cannot tolerate these drugs.
Also indicated for prophylaxis of invasive fungal infections in:
Patients receiving remission-induction chemotherapy for Acute Myelogenous Leukemia (AML) or Myelodysplastic Syndromes (MDS) with expected prolonged neutropenia.
Hematopoietic Stem Cell Transplant (HSCT) recipients undergoing high-dose immunosuppressive therapy for graft-versus-host disease.

👉 Refractoriness is defined as infection progression or no improvement after ≥7 days of effective antifungal therapy.
👉 Use only under the supervision of a registered physician.

✔️ Dosage & Administration | Osapiderm DR 100

Form: Tablet

Administration: Swallow whole with water, with or without food. Do not crush or chew.

Loading Dose (Day 1): 300 mg (3 tablets) twice daily.

Maintenance Dose (Day 2 onwards): 300 mg (3 tablets) once daily.

Duration: Based on disease severity, immunosuppression recovery, and clinical response.

Pediatrics (<13 years): Not recommended (safety not established).

Geriatrics: No dose adjustment required.

Hepatic Impairment: Use with caution; monitor liver function.

Renal Impairment:

Mild–moderate: No adjustment.

Severe: Monitor closely for fungal breakthrough.

✔️ Side Effects | Osapiderm DR 100

Gastrointestinal: Diarrhea, nausea, vomiting

Systemic: Fever, headache, cough

Laboratory: Hypokalemia

✔️ Drug Interactions

CYP3A4 Inhibition: May increase plasma levels of drugs metabolized by CYP3A4 (e.g., Midazolam, Alprazolam, Triazolam).

Avoid co-administration with:

Rifamycins (Rifampicin, Rifabutin)

Anticonvulsants (Phenytoin, Carbamazepine, Phenobarbital, Primidone)

Efavirenz

Contraindicated with ergot alkaloids, certain QT-prolonging drugs (Cisapride, Pimozide, Quinidine, Terfenadine, Astemizole, Halofantrine), and statins (Simvastatin, Lovastatin, Atorvastatin).

✔️ Contraindications

Hypersensitivity to Posaconazole or other azoles.

Concomitant use with drugs that can cause serious QT prolongation or rhabdomyolysis (see above).

✔️ Pregnancy & Lactation

Pregnancy: Not recommended unless benefits outweigh risks.

Lactation: Discontinue breastfeeding during therapy.

✔️ Storage:

Store below 25°C.

Keep out of reach of children.
