✔️ Overdose Management

• Symptoms: Drowsiness, agitation, confusion, respiratory depression, coma
• No specific antidote
• Management: Gastric lavage/emesis, symptomatic treatment, hemodialysis (removes ~60% of the drug)

✔️ Dosage & Administration of Levefix Tablet

Adults & Adolescents (≥16 years)

• Starting dose: 250 mg twice daily
• Maintenance: 500 mg twice daily (after 2 weeks)
• Maximum: 1500 mg twice daily
• Titrate by 250 mg every 2 weeks based on response

Adults (≥18 years) & Adolescents (12–17 years) ≥50 kg

• Start with 500 mg twice daily
• May increase to 1500 mg twice daily
• Dose changes by 500 mg can be made every 2–4 weeks

Pediatrics

• Children <6 years: Use oral solution
• Children 6–11 years or <25 kg: Use solution for accurate dosing
• Children ≥25 kg: Start with 250 mg twice daily, max 750 mg twice daily
• Children ≥50 kg: Same dosage as adults

Elderly: Dose adjustment may be necessary based on kidney function

Children <6 years: Tablets not suitable—use solution

Note: Monotherapy has not been established for children under 16

✔️ Side Effects of Levefix Tablet

Adults: Drowsiness, fatigue, dizziness, infections

Pediatrics: Fatigue, irritability, nasal congestion, aggression, appetite loss

✔️ Drug Interactions

No known interactions with common antiepileptic drugs (e.g., phenytoin, valproate, carbamazepine)

• Probenecid: May reduce renal clearance of metabolite (not clinically significant)
• Methotrexate: May increase methotrexate levels—monitor closely
• Laxatives (macrogol): May reduce Levefix efficacy—avoid within 1 hour before/after dosing
• Food: No effect on extent of absorption; slight delay in onset
• Alcohol: No conclusive data

✔️ Contraindications

Known hypersensitivity to levetiracetam, other pyrrolidone derivatives, or excipients

✔️ Pregnancy & Lactation

Not recommended unless necessary in pregnant or non-contraceptive women of childbearing potential

Excreted in breast milk—avoid breastfeeding unless the benefit outweighs the risk

No adverse effect on fertility was seen in animal studies

✔️ Precautions & Warnings

• Renal Impairment: Dose adjustment required based on renal function
• Liver Impairment: In severe cases, reduce dose if CrCl <60 ml/min
• Blood Dyscrasias: Monitor CBC if the patient has weakness, fever, or recurrent infections
• Suicidality: Watch for suicidal thoughts/behavior; monitor mood
• Acute Kidney Injury: Rare but possible
• Pediatrics: Long-term effects on development are still under study
