✔️ Composition & Description

Active Ingredients:

Budesonide BP: Inhaled corticosteroid with dose-dependent anti-inflammatory action in the airways; reduces symptoms and asthma exacerbations.

Formoterol Fumarate Dihydrate BP: Long-acting β2-adrenergic agonist (LABA) providing rapid (1–3 min) and prolonged (≥12 hours) bronchodilation.

Delivery: Dry powder inhaler (DPI) for oral inhalation only.

Formoterol:

Common: tremor, palpitations, headache

Uncommon/rare: arrhythmias, muscle cramps, hypersensitivity

Budesonide:

Hoarseness, oropharyngeal candida, throat irritation

Rare: cutaneous hypersensitivity, infections, growth suppression in children, glaucoma, cataract

✔️ Dosage & Administration

Asthma – Inhaler (≥12 years):

• Maintenance: 1 puff twice daily or 2 puffs once daily.
• Adjustments: Some may require 2 puffs twice daily (160/4.5 mcg).
• Reliever: 1 additional puff as needed; max 6 puffs per occasion.

Asthma – Inhalation Capsule: 

Adults/Adolescents (≥12 yrs):

• Maintenance: 100 or 200 µg capsule twice daily.
• Reliever: 1 capsule as needed; max 6 per occasion, up to 12/day temporarily.

Children (4+ yrs):

• Maintenance: 100 µg once daily.
• Reliever: 1 capsule as needed; max 4 per occasion, up to 8/day temporarily.

COPD – Inhalation Capsule (≥40 yrs):

200 µg: 2 capsules twice daily (max 4/day).

400 µg: 1 capsule twice daily (max 2/day).

✔️ Administration Technique | Using an Inhaler:

• Remove cap and shake inhaler at least 6 times.
• Prime inhaler if new or unused for ≥1 week.
• Exhale fully; hold inhaler upright.
• Place mouthpiece between teeth, seal lips.
• Inhale deeply while pressing canister; hold breath 10 sec.
• Wait 1 min between puffs if multiple inhalations prescribed.
• Rinse mouth after each use.

Cleaning:

Weekly cleaning recommended: remove canister, rinse actuator & cap (do not submerge canister), dry, reassemble, replace cap.

✔️ Interaction

Avoid strong CYP3A4 inhibitors (e.g., ritonavir).

Use caution with MAO inhibitors, tricyclic antidepressants, beta-blockers, diuretics.

Diuretics may worsen hypokalemia when combined with beta-agonists.

✔️ Contraindications

Acute asthma/COPD requiring intensive care.

Hypersensitivity to any ingredients.

✔️ Pregnancy & Lactation

No adequate clinical data in pregnant women; animal studies show no additional adverse effects from the combination.

Budesonide passes into breast milk; the presence in human milk is unknown.

Use during breastfeeding only if the benefits outweigh the potential risks.

✔️ Precautions & Warnings

Monitor patients with diabetes, thyrotoxicosis, hypokalemia, cardiovascular disease, glaucoma, cataract, and hypercorticism.

Risk of paradoxical bronchospasm; discontinue immediately if it occurs.

In children, monitor growth regularly; adjust corticosteroid dose if growth suppression is observed.

✔️ Storage

• Keep dry, below 25°C, away from light.
• Use inhalation device only; do not swallow capsules.
• Remove capsules from blister just before use to avoid moisture exposure.
• Keep out of reach of children.
