✔️ Uses

Severe acne (nodular or recalcitrant) that is resistant to conventional treatments.

✔️ Consult Your Doctor If You Experience:

• Persistent headache, vomiting, or visual disturbances.
• Severe abdominal pain, rectal bleeding, or jaundice.
• Signs of an allergic reaction (rash, swelling, or difficulty breathing).

✔️ Common Side Effects (>10%):

• Skin-related: Dry skin, chapped lips (cheilitis), rashes, peeling, redness, photosensitivity.
• Systemic: Increased liver enzymes, hypertriglyceridemia, and low HDL cholesterol.
• Musculoskeletal: Joint and muscle pain.
• Ocular: Dry eyes, conjunctivitis.

Rare but Serious Side Effects:

• Severe depression, psychosis, or suicidal ideation.
• Vision issues, such as sudden night blindness.
• Hemorrhagic pancreatitis or severe gastrointestinal issues.
• Premature closure of growth plates in children.

✔️ Dosage & Administration

Adult Dose:

• Initial dose: 0.5–1 mg/kg/day orally in two divided doses for 15–20 weeks.
• In cases of severe acne with scarring, the dose can be increased to 2 mg/kg/day (if tolerated).

Child Dose (>12 years):

• Same as the adult dose. Safety and efficacy are not established in children under 12.

Renal Impairment:

• Start with a reduced dose (e.g., 10 mg daily) and titrate gradually to a maximum of 1 mg/kg/day as needed.

✔️ Drug Interactions:

1. Vitamin A derivatives: Additive toxicity—avoid concurrent use.
2. Tetracyclines: Increased risk of intracranial hypertension.
3. Phenytoin or corticosteroids: Increased risk of osteoporosis.
4. Carbamazepine: Reduced efficacy due to interaction.

✔️ Precautions:

1. Avoid prolonged sun exposure—use sunscreen and protective clothing.
2. Avoid waxing, laser treatments, or harsh exfoliation during and for 6 months post-treatment, as the skin becomes fragile.
3. Do not use tetracyclines concurrently (increased risk of pseudotumor cerebri).
4. Monitor liver function tests (LFTs), triglycerides, and glucose levels periodically.
5. Patients with psychiatric disorders or a history of depression/anorexia nervosa should be closely monitored.
