✔️ How does Tacrocap 1mg Capsule work?

Tacrocap 1 is an immunosuppressive agent. It works by blocking the release of certain chemicals in the body (such as T-cells) that trigger the immune response. It suppresses your body’s immune functions and enables your body to accept the transplanted organ. As a result, it manages the risk of graft rejection and increases the survival of the newly transplanted organ.

✔️ Indication of Tacrocap 1

Tacrocap, an immunosuppressant that inhibits calcineurin, is prescribed to prevent organ rejection in adults and children undergoing liver, kidney, heart, or lung transplants. It is used alongside other immunosuppressive medications.

✔️ Quick Suggestions:

• Eat foods rich in quercetin (a flavonoid), such as apples, cherries, broccoli, spinach, and blueberries.
• Consuming food rich in probiotics helps in developing the immune system against allergies.
• Limit intake of food that might trigger allergies, such as dairy products, soy, eggs, and nuts.
• Avoid consuming foods with excess sugar, as it may flare up inflammation.
• Include fruits, vegetables, whole grains, healthy fats, and fish.
• Avoid contact of your skin with harsh soaps, detergents, cleaning supplies, dust or sand, wool, synthetic fibers, and cigarette smoke.
• Take Tacrocap 1 hour before meals or 2-3 hours after meals or as directed by your physician. Swallow the medicine with a glass of water. Do not crush or chew the medicine.
• Your doctor will decide the correct dose and duration of therapy for you depending on your age, body weight, and disease condition. Do not stop taking Tacrocap without consulting your doctor.

✔️ Dosage and Administration

• Oral doses of Tacrolimus should begin only when oral administration is tolerated.
• In liver, heart, or lung transplant patients, the initial dose can be given 6 hours post-transplant.
• The initial dose may be administered within 24 hours after surgery for kidney transplant patients, provided renal function has stabilized.

Adult Dosage Recommendations

• Kidney Transplant:
  • With azathioprine: 0.2 mg/kg/day (Target trough: 7–20 ng/mL for months 1–3, 5–15 ng/mL for months 4–12).
  • With MMF/IL-2 receptor antagonist: 0.1 mg/kg/day (Target trough: 4–11 ng/mL).
• Liver Transplant: 0.10–0.15 mg/kg/day (Target trough: 5–20 ng/mL).
• Heart Transplant: 0.075 mg/kg/day (Target trough: 10–20 ng/mL for months 1–3, 5–15 ng/mL after month 4).
• Lung Transplant: 0.075 mg/kg/day (Target trough: 10–15 ng/mL for months 1–3, 8–12 ng/mL for months 4–12).

Pediatric Dosage Recommendations

• Kidney Transplant: 0.3 mg/kg/day (Target trough: 5–20 ng/mL).
• Liver Transplant: 0.15–0.2 mg/kg/day (Target trough: 5–20 ng/mL).
• Heart Transplant: 0.3 mg/kg/day (Target trough: 5–20 ng/mL).
• Lung Transplant: 0.3 mg/kg/day (Target trough: 10–20 ng/mL during weeks 1–2, then 10–15 ng/mL until month 12).

Note: Cystic fibrosis patients undergoing lung transplants may require higher doses due to reduced absorption.

✔️ Drug Interactions

• Mycophenolic Acid (MPA): Tacrocap may increase MPA exposure; monitor for side effects and adjust MPA dosage as needed.
• Nelfinavir and Grapefruit Juice: Can raise tacrolimus levels due to CYP3A inhibition; avoid concurrent use.
• CYP3A Inhibitors: Increase tacrolimus levels; monitor concentrations and adjust dosage.
• CYP3A4 Inducers: Lower tacrolimus levels; dosage adjustments may be required.
• Cannabidiol: May necessitate therapeutic monitoring and dose adjustments.

✔️ Contraindications

Tacrocap should not be used in patients allergic to tacrolimus or polyoxyl 60 hydrogenated castor oil (HCO-60).

✔️ Pregnancy and Lactation

Tacrolimus can harm the fetus. Pregnant women should be informed of the risks.

✔️ Precautions and Warnings

• Not interchangeable with extended-release Tacrocap; educate patients to distinguish between the formulations.
• Monitor blood glucose due to the risk of new-onset diabetes post-transplant.
• Monitor kidney function, neurologic symptoms, serum potassium, and blood pressure.
• Avoid live vaccines during treatment.
• Be cautious of drug-induced pure red cell aplasia and thrombotic microangiopathy.
