✔️ Side Effects & Overdose of Tripar 5

Common: Dry mouth, blurred vision, dizziness, nausea, nervousness.
Serious/Rare: Cognitive impairment, urinary retention, constipation, hallucinations, glaucoma, heat intolerance, paralytic ileus, psychiatric disturbances.
Abrupt discontinuation may cause Parkinsonism rebound or Neuroleptic Malignant Syndrome (NMS).

Overdose

Symptoms: Delirium, hallucinations, dry skin, blurred vision, urinary retention, cardiac issues, seizures, coma.
Management: Supportive care, gastric lavage, diazepam for agitation, avoid phenothiazines, maintain hydration and electrolyte balance.

✔️ Dosage & Administration

• Idiopathic Parkinsonism: Start with 1 mg daily, increasing by 2 mg every 3–5 days up to 6–10 mg/day; up to 12–15 mg may be required in postencephalitic cases.
• Drug-induced Parkinsonism: Typically, 5–15 mg/day in divided doses; start with 1 mg and titrate based on response.
• With Levodopa: Use 3–6 mg/day in divided doses, adjusting both drugs to minimize side effects.

Administration Tips: Best taken in divided doses with meals. If dry mouth is an issue, take it before meals; if nausea occurs, take it after meals. Avoid abrupt withdrawal.

✔️ Interactions

• Enhanced sedation may occur when used with alcohol, opioids, barbiturates, or cannabis.
• Increased anticholinergic effects when combined with MAOIs or tricyclic antidepressants.
• May increase the risk of tardive dyskinesia when used with neuroleptics.
• Dose adjustments may be necessary with levodopa to avoid dyskinesia.

✔️ Contraindications

• Hypersensitivity to Trihexyphenidyl.
• Patients with narrow-angle glaucoma (risk of blindness)

✔️ Pregnancy & Lactation

Use only if needed. Unknown if excreted in breast milk. May reduce milk production; use with caution in breastfeeding women.

✔️ Precautions

• Monitor for signs of glaucoma before and during treatment.
• Avoid overheating or dehydration, especially in the elderly or those in hot environments.
• Risk of hyperthermia due to anhidrosis exists.
• Monitor closely for signs of NMS when stopping therapy.
