Antiparkinson drugs (dopamine agonists)

✔️ Dosage & Administration

For Parkinson’s Disease (Adults):

• Initial dose: 0.25 mg three times daily.
• Titration: Adjust weekly based on patient response and tolerance.
• Maximum dose: 24 mg/day (8 mg three times daily).

Titration Schedule:

• Week 1: 0.25 mg TID (0.75 mg/day)
• Week 2: 0.5 mg TID (1.5 mg/day)
• Week 3: 0.75 mg TID (2.25 mg/day)
• Week 4: 1 mg TID (3 mg/day)

Further titration as needed up to max dose.

For Restless Legs Syndrome (Adults):

• Starting dose: 0.25 mg once daily, 1–3 hours before bedtime.
• After 2 days → may increase to 0.5 mg once daily.
• At the end of week 1 → 1 mg once daily.

Further titration based on response, up to a maximum of 4 mg daily.

Suggested Titration (if needed):

• Days 1–2: 0.25 mg once daily
• Days 3–7: 0.5 mg once daily
• Week 2: 1 mg once daily
• Week 3: 1.5 mg once daily
• Week 4: 2 mg once daily
• Week 5: 2.5 mg once daily
• Week 6: 3 mg once daily
• Week 7: 4 mg once daily

✔️ Side Effects

Commonly reported adverse reactions include:

• Neurological: fainting, drowsiness, dizziness, confusion, hallucinations, uncontrolled, sudden movements
• Gastrointestinal: nausea, vomiting, abdominal discomfort, upset stomach
• General: fatigue, headache, increased sweating
• Other: leg swelling, orthostatic hypotension

✔️ Drug Interactions

Dopamine antagonists (e.g., neuroleptics such as phenothiazines, butyrophenones, thioxanthenes, or metoclopramide) may reduce the therapeutic effect of Perkirol.

✔️ Contraindications:

Known hypersensitivity to Ropinirole or any component of the formulation.

✔️ Precautions & Warnings

• Pregnancy Category C – use only if potential benefits outweigh the risks, as safety in pregnancy is not established.
• May inhibit prolactin secretion, potentially reducing lactation.
• Risk of sudden sleep episodes during daily activities.
• May impair blood pressure regulation → risk of orthostatic hypotension.
• Caution in patients with cardiovascular risk or CNS side effects.
