Parkinson’s disease:

Initial: 88 mcg three times daily.

Titrate: Double every 5–7 days up to 350 mcg three times daily.

Max: 3.3 mg/day in 3 divided doses.

During titration, reduce levodopa dose accordingly.

Restless Legs Syndrome (RLS):

Initial: 88 mcg once daily (2–3 hours before bedtime).

Adjust: Double every 4–7 days as needed up to 350 mcg daily.

Max: 540 mcg/day.

Pediatrics: Not recommended in patients under 18 years.

Common adverse effects include:

Neurological: Dizziness, somnolence, sudden sleep onset, confusion, hallucinations, delusions, abnormal dreams, paranoia.

Gastrointestinal: Nausea, constipation.

Cardiovascular: Hypotension, peripheral edema.

Behavioral: Impulse-control disorders (pathological gambling, hypersexuality, binge eating).

Others: Dyskinesia, headache, hyperkinesia, rash/itching, weight gain, libido changes.

Cimetidine, Amantadine → may reduce renal clearance of pramipexole.

Alcohol or sedatives → additive CNS depressive effects.

Minimal interactions as Parixol is not metabolized by P450 enzymes
