✅ Description:
Ranolazine exerts anti-anginal and anti-ischemic effects by inhibiting the late phase of the inward sodium current in heart muscle cells. This results in:
- Decreased intracellular sodium and calcium overload
- Improved myocardial relaxation
- Restoration of ventricular pump function
- Reduced myocardial oxygen consumption
Notably, its effects are not dependent on lowering heart rate or blood pressure. QT interval prolongation occurs due to IKr (potassium current) inhibition, but without an increase in arrhythmic events at therapeutic doses.

Hepatic Impairment:
Mild: Dose adjustment may be necessary
Moderate to severe: Contraindicated
Renal Impairment:
CrCl 30–80 mL/min: Titrate cautiously
CrCl <30 mL/min: Contraindicated

- Use with CYP3A4 inducers (e.g. rifampicin, phenobarbital)

Increased Ranolazine Levels With:
- Moderate CYP3A4 inhibitors (diltiazem, fluconazole)
- P-gp inhibitors (verapamil, cyclosporine)
- CYP2D6 inhibitors (paroxetine)
May Increase Levels Of:
Digoxin, Simvastatin, Atorvastatin
- Other CYP3A4 substrates (e.g. tacrolimus, sirolimus)
- Metformin
Potentially Dangerous With:
- QT-prolonging agents (e.g. dofetilide, quinidine, sotalol)
- CYP3A4 inducers (rifampicin, phenytoin)

- History of cardiac arrhythmias (QT prolongation, bradycardia)
- Electrolyte imbalances (low potassium or magnesium)
- Severe renal impairment
- Use with caution in patients on QT-prolonging drugs
- Not recommended during pregnancy and breastfeeding
