✅ Description:
Isosorbide mononitrate is the main active breakdown product of isosorbide dinitrate and is responsible for most of its effects.
It relaxes blood vessel walls (vascular smooth muscle), which causes widening of veins and arteries — especially veins.
- Vein widening reduces the amount of blood returning to the heart (preload), lowering pressure inside the heart and lungs.
- Artery widening reduces resistance in blood circulation (afterload) and lowers blood pressure.
It also widens the coronary arteries, improving blood supply to the heart muscle.
The exact importance of each of these effects in relieving angina is not fully known.

✔️ Dosage & Administration
Usually 20 mg twice daily — first dose in the morning, second dose 7 hours later. A lower starting dose (10 mg) may be used if needed. Maintenance dose may range from 20–120 mg daily (1–6 tablets), as directed by your doctor.
✔️ Side Effects:
- Headache
- Dizziness
- Low blood pressure (hypotension)
✔️ Do not take Monocard with:
Phosphodiesterase inhibitors (e.g., sildenafil, tadalafil, vardenafil) – may cause dangerous drops in blood pressure.
Riociguat (a guanylate cyclase stimulator).
Other vasodilators or alcohol may increase blood pressure–lowering effects.
Use with calcium channel blockers may cause significant drops in blood pressure; dose adjustments may be needed.
✔️ Do not take Monocard if you:
Are allergic to isosorbide mononitrate.
Are you taking erectile dysfunction drugs (PDE-5 inhibitors) or riociguat?
✔️ Precautions & Warnings
- May cause severe low blood pressure, especially when standing suddenly.
- Use with caution if you are already low in blood volume or have low blood pressure.
- May cause lightheadedness, especially with alcohol.
- Can worsen angina in hypertrophic cardiomyopathy.
- Long-term exposure to nitrates in industrial settings has been linked to tolerance and dependence — relevance to medical use is unclear.
- Headaches are common at the start of treatment; they indicate the medicine is working. Do not change your dosing schedule to avoid headaches — instead, treat them with aspirin or paracetamol.
✔️ Pregnancy & Lactation
Pregnancy: Category B — animal studies show no harm to the fetus, but there are no well-controlled studies in pregnant women. Use only if needed.
Breastfeeding: Unknown if it passes into breast milk. Use with caution.
✔️ Storage:
Store at 20–30 °C, away from light and moisture, and out of reach of children.
