Adults
Nutritional deficiency: 5–10 mcg (200–400 IU) daily.
Osteoporosis (prevention/treatment, >50 years): 800–1000 IU (20–25 mcg) daily, usually with calcium.
Hypoparathyroidism: 50,000–200,000 IU daily with calcium; IM administration is also an option.
Children
0–12 months: 400 IU (10 mcg) daily.
1–18 years: 600 IU (15 mcg) daily.
Vitamin D–resistant rickets: 12,000–500,000 IU daily.
Familial hypophosphatemia: 40,000–80,000 IU daily with phosphate supplements; dose may be adjusted after growth is complete.
Can be taken orally or via intramuscular (IM) injection.
May be taken with or without food.
✔️ Drug Interactions
- Increased hypercalcemia risk: Thiazide diuretics, calcium/phosphate supplements.
- Reduced effectiveness: Rifampicin, isoniazid, ketoconazole, corticosteroids, antiepileptics (e.g., phenytoin).
- Reduced absorption: Cholestyramine, colestipol, mineral oil, orlistat.
- Use with caution: Digoxin or other cardiac glycosides due to arrhythmia risk.

- Hypercalcemia-related disorders.
- Known allergy to colecalciferol or any ingredient in the capsule.
- Evidence of vitamin D toxicity.
✔️ Use with caution in:
Patients with high calcium or phosphate levels.
- Those with heart disease or kidney disorders.
- Breastfeeding mothers (passes into breast milk).
Note: Adequate calcium and magnesium intake is crucial. Deficiency may activate parathyroid hormone (PTH), leading to calcium mobilization from bones.
