Resmetirom is a thyroid hormone receptor-beta (THR-beta) agonist. On March 14, 2024, it was approved by the FDA as the first treatment of liver fibrosis due to noncirrhotic non-alcoholic steatohepatitis (NASH), which is a form of non-alcoholic fatty liver disease (NAFLD).

Thyroid hormones directly regulate lipid metabolism in the liver; thus, impaired thyroid function, such as low serum thyroid hormone levels, is often observed in NAFLD.3 Resmetirom works to reduce liver fat by stimulating fatty acid degradation and oxidation.

Resmetirom is a prescription medication indicated for the treatment of adults with non-cirrhotic nonalcoholic steatohepatitis (NASH) associated with moderate to advanced liver fibrosis. NASH is characterized by excessive fat accumulation in the liver, leading to inflammation and progressive liver damage if left untreated. Resmetirom has demonstrated efficacy in improving liver fibrosis by at least one stage without worsening the NAFLD activity score, as well as achieving resolution of NASH without progression of fibrosis.

The drug works by targeting hepatic lipid metabolism, thereby reducing liver fat accumulation and helping to slow disease progression. Clinical benefits include reductions in hepatic fat content, elevated liver enzyme levels, and fibrosis-related biomarkers in patients with NASH.

Resmetirom is administered orally once daily and may be taken with or without food, as prescribed by a registered physician. The recommended dosage is 80 mg once daily for patients weighing less than 100 kg, and 100 mg once daily for patients weighing 100 kg or more.
