✔️ Side Effects

• Dizziness, fatigue, headache, and weakness.
• Less common:
• Cough, gastrointestinal disturbances, rash, anxiety, and oliguria.
• Rare but serious:
• Angioedema, anaphylaxis, hyperkalemia.

✔️ Dosage & Administration

Dosage should be tailored based on clinical response and tolerance.

For Hypertension:

Initial: 1.25–2.5 mg once daily (if not on a diuretic).

Maintenance: 2.5–20 mg/day (single or divided doses).

Post-MI Heart Failure:

Start within 48 hours: 2.5 mg twice daily.

If hypotension occurs: reduce to 1.25 mg twice daily.

Target: up to 5 mg twice daily.

Cardiovascular Risk Reduction:

Week 1: 2.5 mg once daily.

Weeks 2–4: 5 mg once daily.

Maintenance: up to 10 mg once daily.

Renal Impairment:

Hypertension: Start with 1.25 mg daily, max 5 mg/day.

Heart failure: Start with 1.25 mg daily, may increase up to 2.5 mg twice daily.

Administration: Swallow whole with water. May be taken with or without food. Do not crush or chew.

✔️ Interactions

• Diuretics: May lead to hypotension or hyperkalemia (with potassium-sparing types).
• Lithium: Risk of elevated serum lithium levels.
• NSAIDs: May diminish antihypertensive efficacy and impair renal function.
• Alcohol: Can enhance blood pressure-lowering effects.

✔️ Contraindications

• Known allergy to ramipril or other ACE inhibitors.
• History of angioedema.
• Use with sacubitril/valsartan, aliskiren in diabetic or renally impaired patients.
• Renal artery stenosis, hypotension, or hemodynamic instability.
• Pregnancy and lactation.
• During extracorporeal therapies using negatively charged membranes (e.g., certain dialysis filters).

✔️ Use in Pregnancy & Lactation

Contraindicated in pregnancy due to risk of fetal harm. Avoid in breastfeeding mothers.

✔️ Precautions

Use cautiously in:

• Renal or hepatic impairment
• Electrolyte imbalances (especially hyperkalemia)
• Hypotensive conditions
• Special Populations:
• Elderly: Start at 1.25 mg daily.
• Liver dysfunction: Max dose 2.5 mg/day.
• Renal dysfunction: Adjust dose based on creatinine clearance.
• Prior diuretic use: Consider discontinuation or dose reduction before starting Tritace.

✔️ Storage

Store below 30°C, away from light. Keep out of children’s reach. Use only as prescribed and before the expiry date.
