✔️ Side Effects

Common (especially during early therapy):

Cough, fatigue, weakness, headache, mood, or sleep disturbances

Less Common:

Altered taste, nausea, indigestion, abdominal pain, skin rashes

Mild increase in blood urea/creatinine levels

Occasional proteinuria

Rare:

Angioedema

Decreased hemoglobin, red blood cells, or platelets

✔️ Dosage & Administration

General Rule: Coversyl should be taken once daily, preferably in the morning before meals.

For Hypertension

Start with 4 mg once daily

If needed, increase to 8 mg once daily after 1 month

For Stable Coronary Artery Disease

Begin with 4 mg once daily for 2 weeks

Increase to 8 mg once daily, depending on kidney function and tolerance

Elderly patients: Start with 2 mg daily for one week, then 4 mg the next week before escalating to 8 mg

For Congestive Heart Failure

Initiate under medical supervision with 2 mg once daily

If required, the dose may be increased to 4 mg once daily

Elderly Patients

Start with a low dose (≤4 mg/day)

Increase dosage gradually as needed

Use caution with doses above 8 mg, as clinical data is limited

✔️ Drug Interactions

• With Diuretics: May amplify the blood pressure-lowering effect; stop diuretics 3 days prior to starting Coversyl if possible
• With Potassium Supplements or Potassium-Sparing Diuretics: Increased risk of hyperkalemia
• With Lithium: May elevate lithium levels and toxicity
• With NSAIDs (e.g., ibuprofen, aspirin): May reduce antihypertensive effect and increase risk of kidney impairment
• With Antidiabetics: May heighten the risk of hypoglycemia
• With Gold Compounds (e.g., sodium aurothiomalate): May provoke rare nitritoid reactions

✔️ Contraindications

• Known allergy or hypersensitivity to Perindopril
• Not recommended for children
• Pregnancy and lactation – strictly contraindicated

✔️ Pregnancy & Lactation

Not to be used during pregnancy or while breastfeeding, due to potential risks to the fetus or infant.

✔️ Use with caution in:

• Renal impairment or renovascular hypertension
• Patients undergoing surgery or under anesthesia
• Those with volume depletion or on diuretics (may cause first-dose hypotension)
• Before starting in diuretic-treated patients: Withhold the diuretic 3 days prior if possible
• Avoid combining with potassium-sparing diuretics
• Caution with neuroleptics or tricyclic antidepressants, which may intensify hypotensive effects
• Monitor serum lithium levels if co-administered with lithium
