Special Precautions
- Bronchospastic disease → use cautiously
- Surgery → may impair cardiovascular reflexes

Hypertension: 100–400 mg/day (single or divided doses)
Angina: 50–100 mg, 2–3× daily
Arrhythmias: 50 mg, 2–3× daily (up to 300 mg/day)
Hyperthyroidism: 50 mg, 4× daily (reduce when euthyroid)
MI: IV 5 mg q2min ×3 doses → oral 50 mg q6h for 48 h → maintenance 200 mg/day

- Additive effects with catecholamine-depleting drugs (e.g., reserpine, MAOIs)
- CYP2D6 inhibitors ↑ : metoprolol levels quinidine, fluoxetine, paroxetine, propafenone)
- Digitalis + β-blocker → ↑ bradycardia risk
- May worsen rebound hypertension from clonidine withdrawal

- AV block, uncontrolled heart failure
- Severe bradycardia, sick sinus syndrome
- Cardiogenic shock, severe PAD
- Hypersensitivity to β-blockers
- MI with bradycardia, heart block, hypotension, or severe HF

Common: fatigue, dizziness, depression, diarrhea, rash, bradycardia
Others: insomnia, nausea, constipation, blurred vision, decreased libido, AV block
