Adults:

Initial dose: 5–10 mg once daily
Maintenance: Up to 20 mg daily; Maximum: 40 mg/day

Administration: May be taken with or without food, preferably at the same time daily

Children (10+ years):

Start with 5 mg once daily; may increase to max 20 mg/day

Renal Impairment:

For CrCl <30 mL/min: Max 10 mg/day
Not recommended in severe renal failure, not on dialysis

Interacts with itraconazole, HIV protease inhibitors, gemfibrozil, cyclosporin – increased risk of rhabdomyolysis
