Common:

• Increased bleeding risk.

✔️ Pharmacokinetics:

Absorption:

• Clopidogrel is rapidly absorbed with at least 50% bioavailability.
• Aspirin is well absorbed and reaches peak plasma levels within 30-40 minutes.

Metabolism & Elimination:

• Clopidogrel is metabolized primarily in the liver.
• 50% is excreted in urine, and 46% in feces within 5 days.
• Aspirin is hydrolyzed into salicylic acid and excreted by the kidneys.

✔️ Dosage & Administration: Lopirel Plus

• Standard Dose: Clopidogrel 75 mg once daily with or without food.
• Acute Coronary Syndrome:
  • Loading dose: 300 mg Clopidogrel (one-time).
  • Maintenance dose: 75 mg Clopidogrel daily + Aspirin (75-325 mg) daily.

✔️ Drug Interactions: Lopirel Plus

• Aspirin: Enhances the effect of Clopidogrel but increases the risk of bleeding.
• Heparin: No dose adjustment needed, but increased bleeding risk.
• NSAIDs: Increased risk of gastrointestinal bleeding.
• Warfarin: Increased risk of bleeding. Co-administration should be done with caution.

✔️ Contraindications:

• Hypersensitivity to Clopidogrel, Aspirin, or any component.
• Active bleeding disorders (e.g., Peptic Ulcer, Intracranial Hemorrhage).

✔️ Overdose Management: Lopirel Plus

Clopidogrel Overdose:

• Risk of severe bleeding – platelet transfusion may be required.

Aspirin Overdose:

• Symptoms: Dizziness, headache, tinnitus, confusion, nausea.
• Severe cases: Respiratory/metabolic acidosis, hyperthermia, dehydration.
• Treatment: Activated charcoal, gastric lavage, and hemodialysis if necessary.

✔️ Pregnancy & Lactation:

Pregnancy:

• 1st & 2nd trimesters: Use only if needed.
• 3rd trimester: Contraindicated due to bleeding risk.

Lactation:

• Clopidogrel: Unknown if excreted in human milk.
• Aspirin: Known to be excreted in breast milk – not recommended during breastfeeding.

✔️ Precautions & Warnings:

• May prolong bleeding time – use cautiously in patients with a bleeding risk.
• Monitor for signs of bleeding (black stools, unusual bruising).
