✔️ Side Effects

Commonly reported side effects include gout flares, abnormal liver function tests, diarrhea, nausea, headache, rash, and swelling. In rare cases, serious hypersensitivity reactions may occur.

✔️ Dosage & Administration

• Standard Dosage: The initial dose is 40 mg once daily. If serum uric acid remains above 6 mg/dL after two weeks, the dose may be increased to 80 mg once daily. If levels are still elevated after 2-4 weeks at 80 mg, the dose may be raised to 120 mg once daily.
• Tumor Lysis Syndrome: The recommended dose is 120 mg once daily, starting two days before chemotherapy and continuing for at least seven days (or up to nine days based on clinical assessment).
• Gout Flares: Flares may occur due to fluctuations in serum uric acid levels, leading to the release of urate deposits. Prophylactic use of NSAIDs or colchicine is advised when starting treatment. If a flare occurs, Febuxostat should not be discontinued but managed concurrently.
• Renal & Hepatic Impairment: No dosage adjustment is needed for patients with mild to moderate kidney or liver dysfunction.
• Pediatric Use: Safety and effectiveness have not been established for individuals under 18 years of age.

✔️ Drug Interactions

• Mercaptopurine/Azathioprine: Not recommended due to Febuxostat's mechanism of action, which may elevate plasma levels of these drugs, leading to toxicity.
• Rosiglitazone/CYP2C8 Substrates: No dosage adjustments required.
• NSAIDs (e.g., Naproxen, Probenecid): These may theoretically affect Febuxostat elimination, but co-administration is generally safe.
• Inducers of Glucuronidation: Potent inducers may accelerate Febuxostat metabolism, reducing effectiveness. Serum uric acid levels should be monitored.
• Colchicine, Indomethacin, Hydrochlorothiazide, Warfarin: Co-administration does not require dosage adjustments.
• Antacids: Taking antacids containing magnesium and aluminum hydroxide may delay Febuxostat absorption, but it does not significantly affect overall drug levels.

✔️ Contraindications

Febuxostat should not be used in patients taking azathioprine, mercaptopurine, or theophylline.

✔️ Pregnancy & Lactation

• Pregnancy (Category C): There is insufficient data on its safety during pregnancy. Febuxostat should only be used if the potential benefits outweigh the risks.
• Breastfeeding: It is unknown whether Febuxostat passes into breast milk, so caution is advised when prescribing to nursing mothers.

✔️ Precautions & Warnings

• Gout Flares: Increased flare-ups are common when initiating therapy. Preventive use of NSAIDs or colchicine for up to six months may be beneficial.
• Cardiovascular Risk: Some studies indicate a higher incidence of heart attacks and strokes in patients taking Febuxostat compared to allopurinol. Monitor for symptoms.
• Liver Function: Periodic liver enzyme monitoring is recommended, as elevated transaminase levels have been observed.
