Indications

Ulcers in the upper portion of the small intestine are treated briefly with Seclo DR 20 Tablet (duodenal ulcer). By blocking the organic enzyme system, this medication reduces the amount of acid produced in the stomach. This medication works by lowering the amount of stomach acid. Additionally, Seclo DR 20 is used for the short-term treatment of stomach ulcers, the long-term treatment of cancer of the endocrine gland, the long-term treatment of an increase in the body's mast cell count, the short-term treatment of food pipe inflammation, the long-term treatment of overproduction of gastric acid (Zollinger-Ellison syndrome), and the long-term treatment of heartburn and indigestion caused by GERD (systemic mastocytosis).

As prescribed by the doctor, Seclo DR 20 Tablet may also be taken in combination with other medications to treat specific conditions. It is used in conjunction with clarithromycin or amoxicillin to treat duodenal ulcers, which are located in the upper portion of the small intestine. It is additionally used in conjunction with Metronidazole or Clarithromycin to get rid of the Helicobacter pylori bacteria, which causes stomach ulcers.

Pharmacology

Omeprazole belongs to a class of antisecretory compounds, the substituted benzimidazoles, that suppress gastric acid secretion by specific inhibition of the H+/K+ ATPase enzyme system at the secretory surface of the gastric parietal cell. Because this enzyme system is regarded as the acid (proton) pump within the gastric mucosa, omeprazole has been characterized as a gastric acid-pump inhibitor, in that it blocks the final step of acid production. This effect is dose-related and leads to inhibition of both basal and stimulated acid secretion irrespective of the stimulus.

Dosage

Oral-

Adults typically take 20–40 mg of Seclo Dr. 20 once daily for 4–8 weeks. The typical dosage for children is 5-20 mg once daily (for erosive esophagitis, an inflammation of the food pipe, and acid reflux) (gastroesophageal reflux disease). This medication is typically taken for 8 weeks to treat stomach ulcers, 8 weeks to treat duodenal ulcers, and 4 weeks to treat digestive disorders where stomach acid flows back into the food pipe (gastroesophageal reflux disease). The effects of this medication start appearing after about an hour. There is no history of addiction or habit formation with this medication.
The use of this medication should be based on individual need.

For the long-term management of conditions like excessive stomach acid production, Seclo DR 20 may be prescribed (Zollinger-Ellison Syndrome).

Administration

• Capsule or Tablets should be taken with food. 
• Take it immediately before a meal. 
• Patients should be cautioned that the capsules should not be opened, chewed, or crushed and should be swallowed whole.

Interaction

Warfarin, phenytoin, and diazepam can all be eliminated more slowly after taking omeprazole. When omeprazole is added to the treatment, it is advised to monitor patients who are taking Warfarin or Phenytoin and to possibly reduce the dose of those medications. The metabolism of theophylline or propranolol is unaffected by omeprazole.

Contraindications

Omeprazole is contraindicated in patients with known hypersensitivity to any component of the formulation.

Side Effects

Side effects reported with Omeprazole in clinical studies have included nausea, diarrhea, constipation, flatulence, abdominal colic, paresthesia, dizziness, and headache but are rare. Skin rashes, leukopenia, and transient elevation of plasma activation of hepatic amino-transferases have been observed occasionally in a few patients and there has been no consistent relationship with the treatment.

Pregnancy & Lactation

Pregnancy: There are no adequate and well-controlled studies on the use of Omeprazole in pregnant women. Therapeutic doses during pregnancy are unlikely to pose a substantial teratogenic risk. Omeprazole should be used during pregnancy only if the potential benefit to pregnant women justifies the potential risk to the fetus.

Lactation: Omeprazole is excreted in human milk. Thus, a decision should be taken to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Precautions & Warnings

When a stomach ulcer is suspected, the potential of gastric cancer should be ruled out before starting Omeprazole treatment, as the medication may relieve symptoms while delaying detection.
