Ornova is prescribed for the treatment of:
- Gastric and duodenal ulcers
- Reflux esophagitis
- Prevention of recurrence of gastric or duodenal ulcers in patients taking low-dose aspirin or NSAIDs
- Elderly: Use cautiously due to reduced hepatic/renal function.
- Children (<18 years): Not recommended (safety and efficacy unestablished).
- Renal impairment: No adjustment needed in mild–moderate impairment (eGFR 30–89 mL/min). Avoid in severe impairment (eGFR <30 mL/min).

✅ Description:
Vonoprazan is a potassium-competitive acid blocker (P-CAB) that directly inhibits H+, K+-ATPase in a reversible and potassium-dependent manner, without the need for acid activation. Its strong basicity allows it to localize at the acid-secreting sites of gastric parietal cells, producing potent and long-lasting inhibition of gastric acid secretion. This results in effective protection against upper gastrointestinal mucosal damage.
As part of combination therapy for Helicobacter pylori eradication in conditions such as gastric/duodenal ulcer, gastric MALT lymphoma, idiopathic thrombocytopenic purpura, post-endoscopic resection of early gastric cancer, or H. pylori gastritis.
- Use during pregnancy only if the benefits clearly outweigh potential risks.
- Breastfeeding is not recommended; discontinue nursing if treatment is essential.
- Use the lowest effective dose and monitor disease progression closely.
- Long-term use should be evaluated periodically (e.g., via endoscopy).
- In reflux esophagitis, maintenance therapy should be reserved for patients with recurrent disease.
- Consider dose reduction (from 20 mg to 10 mg) or discontinuation once healing is achieved and recurrence risk is low.

Possible adverse reactions include:
- Gastrointestinal: Diarrhea, constipation, nausea, abdominal bloating
- Hypersensitivity reactions: Rash, urticaria, drug eruption, anaphylaxis (rare)

Gastric ulcer: 20 mg once daily for 8 weeks
Duodenal ulcer: 20 mg once daily for 6 weeks
Reflux esophagitis: 20 mg once daily for 4 weeks (extendable to 8 weeks if needed)
Maintenance of reflux esophagitis: 10 mg once daily (may be increased to 20 mg if required)
Prevention of ulcer recurrence (aspirin or NSAID use): 10 mg once daily
H. pylori eradication (first-line): 20 mg vonoprazan + 750 mg amoxicillin + 200 mg clarithromycin, all taken twice daily for 7 days (clarithromycin may be increased up to 400 mg twice daily if necessary).
H. pylori eradication (alternative): 20 mg vonoprazan + 750 mg amoxicillin + 250 mg metronidazole, all taken twice daily for 7 days.
Ornova can be taken with or without food.
✔️ Contraindications (ব্যবহার নিষেধ):
Known hypersensitivity to vonoprazan or any component of the formulation
Concomitant use with atazanavir, nelfinavir, or rilpivirine
