✔️ Uses of Topmate 50

✔️ Pharmacology:

• Topmate is a sulfamate-substituted monosaccharide, which impacts GABA receptors to enhance inhibitory effects and reduces excitatory neurotransmission through AMPA and kainate receptors.
• It also weakly inhibits carbonic anhydrase, which can contribute to side effects like a metallic taste or renal stones.

✔️ Dosage: Topmate 50

• Epilepsy:
  • Monotherapy for Adults: Start with 25 mg daily at night, gradually increasing to 100 mg/day, with a maximum dose of 400 mg/day.
  • Adjunctive Therapy for Adults: Begin at 200 mg/day, up to 400 mg/day, divided into two doses.
  • Pediatric Dosages vary by age and response, often starting low and titrated upwards.
• Migraine Prophylaxis: Begin with 25 mg nightly, gradually increasing to a target dose of 100 mg/day, divided into two doses.

✔️ Administration: Topmate 50

Topmate 50 mg can be taken with or without food.

✔️ Side Effects:

Common side effects include nausea, abdominal pain, dry mouth, taste disturbances, weight loss, fatigue, drowsiness, and memory issues. Rarely, severe reactions like metabolic acidosis, leukopenia, and thrombocytopenia may occur.

✔️ Interactions:

• Topmate may affect the blood levels of some antiepileptic drugs (e.g., phenytoin).
• It can reduce the efficacy of oral contraceptives containing estrogen.
• It should be used cautiously with CNS depressants, alcohol, and drugs that increase the risk of nephrolithiasis (kidney stones).

✔️ Contraindications:

• Hypersensitivity to any component of Topmate 50 mg.

✔️ Pregnancy & Lactation:

Topmate is a pregnancy category C drug; it should be used during pregnancy only if the potential benefit outweighs the risks. Breastfeeding mothers should exercise caution as the drug's excretion in breast milk is not well studied.

✔️ Precautions:

• Topmate should be withdrawn gradually to avoid triggering seizures.
• Patients should be cautious when driving or operating machinery, as Topmate can cause drowsiness or visual disturbances.

✔️ Storage:

Store below 30°C, away from light and moisture, and out of children’s reach.
