✔️ Side Effects:

• Common side effects: Nausea, vomiting, constipation, somnolence, dizziness, fatigue, coordination and balance disturbances, and irritability.

✔️ Dosage & Administration

• Adults (16 years and older):
  • Initial dose: 50 mg twice daily (100 mg/day).
  • Maintenance dose: 25 mg to 100 mg twice daily (50 mg to 200 mg/day).
• Pediatric patients (50 kg or more):
  • Initial dose: 25 mg to 50 mg twice daily (50 mg to 100 mg/day).
  • Maintenance dose: 25 mg to 100 mg twice daily (50 mg to 200 mg/day).
• Pediatric patients (20 kg to less than 50 kg):
  • Initial dose: 0.5 mg/kg to 1 mg/kg twice daily (1 mg/kg to 2 mg/kg/day).
  • Maintenance dose: 0.5 mg/kg to 2 mg/kg twice daily (1 mg/kg to 4 mg/kg/day).
• Pediatric patients (11 kg to less than 20 kg):
  • Initial dose: 0.5 mg/kg to 1.25 mg/kg twice daily (1 mg/kg to 2.5 mg/kg/day).
  • Maintenance dose: 0.5 mg/kg to 2.5 mg/kg twice daily (1 mg/kg to 5 mg/kg/day).
• Pediatric patients (less than 11 kg):
  • Initial dose: 0.75 mg/kg to 1.5 mg/kg twice daily (1.5 mg/kg to 3 mg/kg/day).
  • Maintenance dose: 0.75 mg/kg to 3 mg/kg twice daily (1.5 mg/kg to 6 mg/kg/day).
• Geriatric Use: Start at the low end of the dosing range.
• Renal Impairment: No dose adjustment needed except for patients with end-stage renal disease undergoing dialysis.

✔️ Drug Interactions:

• Rifampin: May decrease Epicure plasma concentrations; increase dose by up to 100%.
• Carbamazepine: May increase exposure to carbamazepine-epoxide; dose reduction may be needed.
• Phenytoin: Monitor phenytoin levels as Epicure may increase plasma concentrations.
• Levetiracetam: No added benefit when used with Brivaracetam.

✔️ Contraindications:

Contraindicated in patients with hypersensitivity to Brivaracetam or any other components of the product.

✔️ Pregnancy & Lactation:

• Pregnancy: Only use if the benefit to the mother outweighs the risk to the fetus.
• Lactation: Unknown if Brivaracetam is excreted in human milk. Weigh the benefits of breastfeeding against potential risks.

✔️ Precautions & Warnings:

• Suicidal Thoughts/Behavior: AEDs like Brivaracetam increase the risk; monitor for mood changes and suicidal thoughts.
• Somnolence & Coordination Disturbances: May cause dizziness, fatigue, and coordination issues; avoid driving or operating machinery.
• Psychiatric Reactions: Includes irritability, anxiety, aggression, psychosis, hallucinations, and mood swings.
• Gradual Withdrawal: To avoid increased seizure frequency, it should be withdrawn gradually.
