✔️ Dosage & Administration of Vintelix 5

• Starting Dose: 10 mg orally once daily, with or without food.
• Dose Adjustment: May be increased to 20 mg/day if tolerated.
• Minimum Dose: For patients with intolerance, reduce to 5 mg/day.
• Elderly Patients (>65 years): Begin with the lowest dose of 5 mg/day.
• Maintenance Treatment: Long-term use may be required; reassess periodically.
• Discontinuation: For patients on 15–20 mg/day, reduce to 10 mg/day for one week before stopping to reduce withdrawal symptoms such as headache and muscle tension.
• Switching from/to MAOIs: Allow a 14-day gap when transitioning from MAOIs to Vintelix and a 21-day interval when switching from Vintelix to MAOIs.

Special Populations

• Pediatrics (<18 years): Safety and efficacy not established.
• Elderly (>65 years): No dose adjustment needed beyond starting at 5 mg/day.
• Renal Impairment: No dose adjustment required.
• Hepatic Impairment: Not recommended in severe impairment; no dose change for mild to moderate cases.
• CYP2D6 Poor Metabolizers: Vortioxetine exposure may be doubled—adjust dose if used with strong CYP3A4/2C9 inhibitors.
• No dose adjustments necessary based on race, sex, or ethnicity.

✔️ Adverse Effects of Vintelix 5

Possible side effects include:

• Hypersensitivity reactions
• Suicidal thoughts or behavioral changes
• Serotonin syndrome
• Abnormal bleeding
• Mania or hypomania activation
• Angle-closure glaucoma
• Hyponatremia (low sodium levels)

✔️ Drug Interactions

• MAO Inhibitors: Risk of severe or fatal reactions if combined; contraindicated.
• Serotonergic Agents: Combining with SSRIs, SNRIs, Tramadol, Triptans, Buspirone, or Tryptophan may trigger serotonin syndrome. Discontinue both drugs immediately if symptoms occur.
• Lithium: No significant interaction observed.
• CYP2D6 Inhibitors (e.g., Bupropion, Fluoxetine, Paroxetine): Reduce Vintelix dose by 50%.
• CYP Inducers (e.g., Rifampin, Phenytoin, Carbamazepine): Consider dose increase of Vintelix.

✔️ Use in Pregnancy and Lactation

• Pregnancy: Not recommended unless benefits outweigh risks.
• Lactation: Vortioxetine and its metabolites may pass into breast milk. Breastfeeding is discouraged during therapy due to unknown risks to the infant.

✔️ Warnings and Precautions

• Suicidality: Monitor all patients for mood worsening or suicidal behavior, especially in early treatment or during dose changes.
• Serotonin Syndrome: May occur when used alone or with serotonergic or MAOI agents; discontinue immediately if symptoms develop.
• Bleeding Risk: Use cautiously with anticoagulants (e.g., Warfarin), NSAIDs, or Aspirin.
• Mania/Hypomania: Use cautiously in patients with a history or family history of bipolar disorder.
• Angle-Closure Glaucoma: Pupillary dilation may trigger attacks in patients with narrow angles.
• Hyponatremia: Monitor elderly and those on diuretics or prone to low sodium.
