✔️ Common Side Effects

• Gastrointestinal: Nausea, vomiting, diarrhea
• Infections: Upper respiratory tract infection, urinary tract infection
• Liver: Elevated liver enzymes (ALT, AST)
• Musculoskeletal: Muscle pain (myalgia)
• Others: Headache, fatigue, rash, itching, insomnia

✔️ Dosage & Administration

General Instructions:

• Take on an empty stomach: either 1 hour before or 2 hours after food.
• Avoid dairy products, antacids, or mineral supplements (e.g. calcium, magnesium, iron, aluminum, selenium, zinc) within 4 hours before or after dosing.

Adult Dose:

• Chronic ITP & HCV-related thrombocytopenia: Start with 50 mg once daily; adjust to maintain platelet counts ≥50×10⁹/L. Max 100 mg/day.
• Severe Aplastic Anemia (SAA): Start with 50 mg once daily; titrate based on platelet response. Do not exceed 150 mg/day.
• Hepatic impairment: Avoid in patients with cirrhosis unless the benefits outweigh risks (e.g. portal vein thrombosis).

Pediatric Dose:

• Ages 6 years and older: 50 mg once daily.
• Ages 1–5 years: 25 mg once daily.

Dose is adjusted based on platelet count response.

✔️ Drug Interactions

• Reduced absorption with antacids, calcium-containing foods, or supplements with polyvalent cations.
• Increased eltrombopag levels with fluvoxamine; decreased with rifampicin, lopinavir/ritonavir.
• Additive effects with HMG-CoA reductase inhibitors (monitor for toxicity).

High-fat meals significantly reduce bioavailability; take on an empty stomach.

✔️ Pregnancy & Lactation

Pregnancy Category: Not recommended. Animal studies show fetal toxicity. Use only if the benefits clearly outweigh the risks.

Lactation: Not advised. Unknown if excreted in human milk, but detected in animal studies.

✔️ Precautions

• Monitor platelet counts, liver enzymes, and blood chemistry regularly.
• Caution in patients with hepatic impairment, as the risk of portal vein thrombosis may increase.
• Discontinuation may cause rebound thrombocytopenia, increasing the bleeding risk.
• Risk of bone marrow reticulin formation and potential fibrosis.
• Possible increased risk of thromboembolism at high platelet levels.
• Monitor for disease progression in hematologic malignancies.
• Use effective contraception in females of reproductive potential during treatment and for at least 7 days after stopping therapy.
