✔️ Dosage & Administration
Route: Oral; take with or without food. For oral solution, may be taken with water/soft drink to mask bitterness.
Adults
- Cortical myoclonus (adjunct): Start 7.2 g/day in 2–3 divided doses. Increase by 4.8 g every 3–4 days; max 20 g/day.
- Cognitive enhancer / cerebrocortical insufficiency: 2.4 g/day in 2–3 doses; up to 4.8 g/day in severe cases.
Children
- <16 years: Not recommended.
- ≥16 years: 50 mg/kg/day in 3 divided doses; reduce to half once the desired effect is achieved.
Renal Impairment (dose adjustment by CrCl):
- 50–79 mL/min → ⅔ of usual dose
- 30–49 mL/min → ⅓ of usual dose
- 20–29 mL/min → ⅙ of usual dose
- <20 mL/min → Contraindicated

- CNS: Nervousness, hyperkinesia, agitation, insomnia, somnolence, dizziness, depression, abnormal voluntary movements

- General: Weight gain, rash, hypersensitivity reactions
✔️ Drug Interactions
Warfarin: May prolong prothrombin time → monitor INR.
Concomitant use with platelet inhibitors or anticoagulants → increased bleeding risk.
✔️ Precautions & Warnings
- Avoid abrupt discontinuation (may worsen myoclonus, trigger relapse).
- Use caution in renal impairment, cardiac disorders, hemostatic abnormalities, elderly patients, and post-surgical cases.
- May prolong prothrombin time in patients taking warfarin.
- Can cause dizziness and sleepiness → avoid driving/operating machinery until effects are known.
