✔️ Side Effects: Prednisolone

Fluid/electrolyte: Sodium/fluid retention, potassium loss, hypertension, CHF in predisposed patients.

Musculoskeletal: Weakness, osteoporosis, aseptic necrosis, fractures.

GI: Peptic ulcer, GI bleeding, pancreatitis.

Dermatologic: Fragile skin, poor wound healing, petechiae, erythema.

Neurological: Headache, vertigo, pseudotumor cerebri, seizures.

Endocrine: Cushingoid features, growth suppression, menstrual irregularities, glucose intolerance.

Ophthalmic: Cataracts, glaucoma, exophthalmos.

Metabolic: Negative nitrogen balance.

✔️ Dosage & Administration | Prednisolone

Adults

Nephrotic syndrome:

Initial: 2 mg/kg/day (max 80 mg/day) in 3–4 divided doses until proteinuria resolves for 3 consecutive days (max 28 days).

Then: 1–1.5 mg/kg every other day for 4 weeks.

Maintenance: 0.5–1 mg/kg every other day for 3–6 months.

Anti-inflammatory: 5–60 mg/day in 1–4 divided doses.

Acute asthma: 40–60 mg/day PO (single or divided) for 3–10 days.

Allergic conditions (tapering regimen):

Day 1: 10 mg (before breakfast), 5 mg (after lunch & dinner), 10 mg (bedtime)

Gradual dose reduction until Day 6: 5 mg (morning only).

Pediatrics

Asthma (Acute/Maintenance):

1 year: 10 mg q12h / every other day

1–4 years: 20 mg q12h / every other day

5–12 years: 30 mg q12h / every other day

≥12 years: 40 mg q12h / every other day

Anti-inflammatory / Immunosuppression: 0.05–2 mg/kg/day in 1–4 divided doses.

✔️ Interaction

Reduced efficacy with aminoglutethimide, antacids, barbiturates, carbamazepine, griseofulvin, mitotane, phenylbutazone, phenytoin, primidone, and rifampin.

Risk of arrhythmia with digitalis if hypokalemia develops.

Use caution with immunizations during therapy.

✔️ Contraindications

• Systemic infections without appropriate anti-infective cover.
• Hypersensitivity to prednisolone or formulation components.
• Ocular herpes simplex (risk of corneal perforation).

✔️ Precautions & Warnings

• Avoid exposure to chickenpox/measles; seek immediate medical advice if exposed.
• An increased dose may be needed during stress (surgery, trauma, severe illness).
• Can mask signs of infection and reduce immunity.
• Avoid smallpox vaccination; other vaccines may be less effective or risky.
• Monitor growth in children on long-term therapy.
